Only electronic bid bonds submitted through the designated electronic medium, either Surety2000 or Tinubu Surety Solution, will be accepted with the bid. A fully executed performance bond shall be included with all bids, except on contracts for roadside mowing, vegetation control, demolition, landscaping, fencing, seeding and sodding, bridge painting, and guardrail installations of $100,000 or less, when the bidder may elect not to furnish a performance bond. Electronically does not support submittal of the performance bond. The performance bond must be submitted to the Construction Letting email inbox or be delivered on paper by the time set for receipt of proposal bids. Each contractor that submits a proposal bid in the amount of $150,000 or more for highway improvements described in this notice is required to submit with that bid a written plan for a program to test the contractor's employees for drugs. The plan shall be in compliance with Indiana Code 4-13-18. The plan must be submitted to the Construction Letting email inbox or deposited with the INDOT Prequalification Engineer by the time set for receipt of proposal bids. INDOT - Call 191 - Pavement Replacement and Utility Relocation The subsurface conditions for the proposed project were investigated by ATC and drilling was performed with both truck-mounted and all-terrain-vehicle-mounted drilling equipment using hollowstem-auger methods to advance the boreholes. Split-barrel samples were obtained using standard penetration test (SPT) procedures (American Association of State Highway and Transportation Officials-AASHTO-Method T206) at 2.5 ft to 5.0 ft intervals. The test boring locations were staked in the field by ATC representatives, with approximate boring ground surface elevations, stations and offsets estimated from the Stage 2 plans provided by American Structurepoint. The test borings were drilled at the approximate locations noted on the Test Boring Logs in Appendix B and as depicted on the Boring Plans (Figures 3 through 5 in Appendix A). Logs of all borings, which show visual descriptions of all soil strata encountered using the AASHTO classification system are included in Appendix B. Sampling information and other pertinent field data and observations are also included on the boring logs. In addition, a sheet defining the terms and symbols used on the logs and explaining the SPT procedure is provided immediately preceding the test boring logs in Appendix B. Pavement cores were collected at six of the test boring locations and photographs were obtained of the core location and the retained pavement core. These pavement core locations and photographs were uploaded to the state wide ArcGIS database utilizing the INDOT/ESRI Collector. Pavement Core Logs are included in Appendix G.
