Annual Inspection & Repair of Aerial Units, Buckets, and Lifts. Question Deadline 08/15/2024 at 4:00 PM ET Questions must be submitted at online. The City of Toledo encourages all bidders to achieve a 10% MBE participation on all goods & services contracts. The MBE companies must be registered with the City of Toledo for these percentages to count towards this goal. In order to be responsive, all bids must have the following with submittal: o Project Paperwork* o Line Items - will auto-populate with e-bid instructions below o Signed Contract by the Vendor* o Child Support Affidavit* o Bureau of Worker's Compensation Certificate Preferences MBE - Minority-Owned Business Enterprise WBE - Woman-Owned Business Enterprise Bid Bond 5.00% Payment Bond 0.00% Performance Bond 100.00% Bid Guaranty: bid bond or certified/cashier's check on a solvent bank; see instructions below. Project Duration 1-year contract, with option for 3 one-year renewals Bid Guaranty requirements are located on the "Bid Information" tab on . The bid guaranty shall be in the form of a cashier's check/official check on a solvent bank or a bid bond. If using a check as the bid guaranty, the check must be received by the Division of Purchases and Supplies no later than the due date and time to be deemed responsive. The check must also be scanned and uploaded with the e-bid. Company checks and personal checks will NOT be accepted. Checks shall be made payable to "City of Toledo. Scope of Services The City of Toledo is accepting bids for annual inspection of Aerial Units, Buckets and Lifts As noted above, City forms must be filled out and attached to the bid. Project Paperwork includes the following: Acknowledgment and Acceptance of Conditions Contract Specifications; Toledo Municipal Income Tax Compliance; Public Utilities Compliance; MBE Good Faith and Goal Commitment; EEO Form; Primary and Subcontractor Forms and Debarment and Suspension Certificate; Best Bid Criteria. Other required forms include those noted in the Response Type Section and also noted in the Other Details Section: Signed Contract by Vendor CEO/President; Child Support Affidavit; Insurance Certificate listing the city of Toledo as an additional insurer; and Current Worker's Compensation Certificate. The following items will be required upon awarding the contract: Performance Bond and PLA, if applicable.
