The work to be performed under this contract consists of furnishing all labor, equipment, tools and materials for the repairs to Holland Road between Engle Road and Eastland Road, in Brook Park, Ohio, which may include asphaltic resurfacing, joint repair, replacement of defective base, concrete curb and aprons, adjustment of castings and repairs of defective catch basins. The Contractor shall include the specification booklet along with any addenda, in their entirety and original form, with his/her bid. All bidding documents must be obtained from the offices of Euthenics, Inc., duplications of the Bid Form and Contract Documents will be considered unacceptable. Each bid must be made upon the Bid Form provided in the contract documents and must be accompanied by a bid bond in the amount of 100% of the amount bid or a certified or cashier's check or letter of credit drawn on a solvent bank in the State of Ohio in an amount equal to ten percent (10%) of the amount bid as a guarantee that if said bid is accepted, a contract will be entered into and a Surety Bond satisfactory to said City in an amount equal to 100% of the total bid will be furnished. Checks shall be made payable to the City of Brook Park, Ohio. Bids will be opened and read at the time and place stated above. Those who have submitted proposals or their representatives are invited to attend. The regulations of the State of Ohio relative to the procurement of labor, wage rates and hours of employment shall apply.
