Site work and paving for a road / highway in Suffern, New York. Completed plans call for site work for a road / highway; and for paving for a road / highway.

Hudson Valley DDSO reserves the right to reject any quote which does not reflect the correct prevailing wages in accordance with the provided wage schedule. Tabulation of quotes will be posted to the listings on NYS Contract Reporter along with the award. Driveway Resurfacing The vendor shall provide all labor and materials necessary to complete the work in a professional manner according to scope of work and architectural drawings if supplied. Work is to be completed within 30 days ofaward, unless modified below. This is a NYS Public Works project and prevailing wages are to be paid. Refer to attached specification for the following scope of work. Site must be cleaned and all materials secured at the end of each work day. Absolutely no scraps, especially fasteners, are to be left unsecured. Upon completion of Driveway Re-surfacing, before removal of equipment and scaffolding, contractor is to schedule a quality control and quality assurance inspection with Plant Superintendent SCOPE OF WORK includes, but is not necessarily limited to the following: 1. The vendor is paving approximately 7,000 sq. feet. See diagram for reference. 2. The vendor is responsible to verify all measurements. 3. The vendor will grind down top layer of existing driveway. 4. The vendor will be required to excavate, and grade/ slope the entire area of the driveway for proper drainage as needed. Add and compact sub-base as needed. 5. The vendor will supply and apply a layer oftack coat on grinded driveway. 6. The vendor will pave a layer of HMA finish/ surface coarse at minimum thickness of two (2) inches. 7. The vendor is responsible for smooth transition/ grading of butt joints to roadway, walkway, and connecting driveway. 8. HVDDSO will provide the item# 4 stone and asphalt for this project. 9. The vendor shall provide all required trucking of materials (i.e. Item#4, asphalt) delivered to site, install all materials as required for this project and follow manufacturer's instruction for placement of materials and industry standards. 10. The Vendor shall submit all pick up/ delivery tickets to HVDDSO Work Control representative. 11. The vendor will supply all pick up/ delivery tickets included with invoice at the completion of project. Failure to do so will delay in payment. The sign in sheet is a separate entity from the resident visitor's log. When signing in, be sure to check the project name listed on the sign in sheet to ensure you are recording your attendance correctly. Failure to do so will cause out to be ineligible to submit a quote. There are no exceptions. Please write clearly for corresponding purposes and make sure you include an email address and phone number where you can be reached. Late arrivers at the site will not be allowed to sign in, participate, nor submit a quote for this project. If you are unable to attend either day, you will not be able to schedule a site visit at different time.
