The Ashtabula County Solid Waste Management District (hereinafter referred to as the "District") is soliciting qualified contractors to provide services for proper collection, transportation, recycling and/or disposal of household hazardous waste ("HHW") which is comprised of the materials set forth in the Pricing Sheet attached hereto as Attachment A. The objective of the collection events is to provide households in the District with an opportunity to turn in HHW for proper recycling and/or disposal. Information pertaining to the District's proposed collection event(s) and past collection results can be found in Attachments B to this RFQ. The successful respondent will be required to furnish a performance bond or other surety method approvable by the Ashtabula County Prosecutor to the District in the amount of 100% of Contract Price, the condition of which shall be the full and complete execution and performance of each and all terms contained in the Agreement set forth in Attachment C hereto. The entire cost of the bond(s) shall be paid for by the contractor. The Contract Price the District performance bond shall be the "not to exceed" amount set forth in Section 1 of Article II of Attachment C. The contractor must possess the necessary experience, expertise, knowledge, ability, staffing, and permits to conduct handling, identification, recycling and/or disposal of hazardous waste. The selected firm will provide all expertise, labor, packing materials, packing containers, trucking containers, equipment, services, and performance bond to fulfill all terms and conditions of the awarded contract set forth in Attachment C hereto. The firm must be able to fulfill all terms and conditions without sub contractors except for transportation, recycling, and disposal services. The firm must have the capacity to conduct multiple collection events on the same day if required. A preliminary schedule of 3 collection events is provided in Attachment B. Qualified firms are requested to respond to this RFQ if they can provide services of collection site management, storage, identification, packaging, labeling, transporting and record-keeping related to the safe and proper collection and shipment of the HHW to an approved recycling or treatment, storage or disposal facility (TSDF). Respondent quotes which adequately address all items listed under "Article IV Quote Requirements" will be considered for a contract award. The award will be based on the consideration of all the items listed under "Article VI. Quote Evaluation". The selected contractor will be required to enter into an agreement with the Districts in substantially the form set forth in Attachment C hereto. The District reserves the right to reject any and all bids. Any quote submitted in response to this RFQ may be rejected from consideration by the District if: 1. The quote does not contain substantially all information as requested in this RFQ as necessary for the evaluation as indicated above; or 2. The person or firm has been suspended, debarred or voluntarily excluded under 40 CF Part 32; or 3. The quote does not demonstrate the contractor's ability to perform the work tasks as specified.
