Renovation of a medical facility in Bowling Green, Ohio. Completed plans call for the renovation of a medical facility.

Wood County Health Department Renovations The work will consist of one (1) single prime contract. The estimate of probable cost for the project is $1,853,000.00, with Alternate 1-$40,000, Alternate 2-$25,000 Alternate 3-$45,000, Alternate 4- $35,000 and Alternate 5-$4,500. Bidders will not be responsible for paying sales taxes on materials incorporated into the structure. The Wood County Combined General Health District is tax exempt and a certificate will be provided to the successful contractor, The Health and Preparedness Building portion of this project will be partially funded by American Rescue Plan Act (ARPA) funds, and partially by the Public Health Emergency Response: Cooperative Agreement for Emergency Response: Public Health Crisis Response (CFDA 93.354) of the United States Department of Health and Human Services (HHS), Grant Pass-Through 08710012WF0223 funds, in the amount of $149,999 . The portion of the project funded with CFDA 93.354 must be completed by July 31, 2025. All Requests for Information (RFI) and questions must be e-mailed (no telephone calis) to hjudge@ssoe.com and received by September 6, 2024 by 2 p.m. An addendum will be issued September 10, 2024 by 2:00 p.m. in response to all received RFIs. The bid guaranty may be of two forms: 1. A Bid Guaranty and Contract Bond using the form in the Contract Documents. 2. A certified check, cashier's check or letter of credit in favor of Wood County Commissioners, in the amount of ten percent (10%) of the bid. If the contract is awarded, a Contract Bond will be required, which is a one hundred percent (100%) payment and performance bond. The successful bidder will be required to pay not less than the minimum wage rates established by the United States Department of Labor, Wage and Hour Division. In accordance with all provisions of the Davis Bacon and Related Acts, 40 U.S.C. 3141-3148, the wage rates and fringe benefits in the applicable Davis-Bacon wage determination shall be the minimum paid by contractor and subcontractors to laborers and mechanics on projects covered by the Davis-Bacon Act and issued by the State of Ohio Department of Commerce, Division of Labor and Worker Safety, Wage and Hour Bureau, pursuant to Ohio Revised Code Chapter 4115. The Board of County Commissioners may reject any or all bids. The Board of County Commissioners reserves the right to waive any irregularities in the form of the bid that do not affect or destroy competitive bidding. In no case will an award be made until all necessary investigations are made as to the qualifications of the bidder to whom it is proposed to award the contract. No bidder may withdraw his bid for a period of sixty (60) days after the scheduled closing time for the receipt of bids. All construction services required for the renovation of 11,500 square feet of spaces serving Environmental Health, Health Promotion & Preparedness, Nutrition Division (WIC), support and administrative spaces, and other Work indicated in the Contract Documents including: a. Rough Carpentry, Architectural Woodwork, laminate clad casework, solid surface countertops and all blocking and necessary fastening and anchoring; b. Insulation, joint sealants; c. Doors and frames, transaction windows and associated glazing, hardware, coordinating with card readers, raceways, and power source requirements for a complete operating system; d. Gypsum board, floor coverings including preparation of floor for leveling especially at cut slab for new plumbing, wood veneer surfaces, acoustical ceilings, and painting, including priming and finish coats of all interior exposed surfaces, surface acceptance, protection of areas not in contract; e. Wall covering, wall surface protection and tackboard; f. Installation of owner-supplied signage, paper towel dispenser and other accessories; g. Selective demolition for installation of new plumbing and other trades to install all associated work including all accessories for a complete plumbing installationosanitary, vent, domestic water; follow owner standards for piping labeling. h. Fire Protection relocation and new as required for new layout; i. Selective demolition and replace in kind in areas outside of project scope area to install new HVAC, including redundant cooling for the Server Room, fans, VAVs, ductwork, diffusers/grilles, all controls, and thermostat locations approved by owner. Follow owneris requirements for protection of existing ductwork and prevention of dust and debris entering system; j. Selective Demolition of power, data, and light fixtures per drawings including floor cutting and existing wall cutting and patching; new outlets and light fixtures with occupancy sensors as shown on drawings; coordinate with owneris furniture installation for power and data at specified locations; include power for new HVAC;
