One (1) Completed Copies of the Illinois Department of Human Rights Form Pc-2, Entitled "Bidder's Employee Utilization Form Construction". 3. Form a or Form B: Certifications & Additional Terms, Disclosures and Conflicts of Interest Vendor/contractor Use of Subcontractors Bids May Not Be Withdrawn After the Opening of Bids.
