The City of Toledo Police Department is interested in upgrading the security system for the Toledo Police Tow Lot at 198 Dura Ave Toledo, OH 43612. This project is in conjunction with another project and cumulatively could trigger the need for a PLA. Question Deadline 09/26/2024 at 4:00 PM ET Online Q&A Yes Bid Bond 2.5% Payment Bond 0.00% Performance Bond 100.00% BID SUBMISSION In order to be responsive, all bids must have the following uploaded with submittal in online: o Project Paperwork* o Signed Contract by the Vendor* o Child Support Affidavit* o Pricing Sheet ** (will auto-populate with e-bid) o Insurance Certificate, naming the City of Toledo as an additional insured / Certificate Holder o Current Bureau of Worker's Compensation Certificate As noted above, City forms must be filled out and attached to the bid. Project Paperwork includes the following: Acknowledgment and Acceptance of Conditions Contract Specifications; Toledo Municipal Income Tax Compliance; Public Utilities Compliance; MBE Good Faith and Goal Commitment; EEO Form; Primary and Subcontractor Forms and Debarment and Suspension Certificate; Best Bid Criteria. Other required forms include those noted in the Response Type Section and also noted in the Other Details Section: Signed Contract by Vendor CEO/President; Child Support Affidavit; Insurance Certificate listing the city of Toledo as an additional insurer; and Current Worker's Compensation Certificate. The following items will be required upon awarding the contract: Performance Bond and PLA, if applicable. o Bid Guaranty: bid bond or certified/cashier's check on a solvent bank The bid guaranty shall be in the form of a cashier's check/official check on a solvent bank or a bid bond. If using a check as the bid guaranty, the check must be received by the Division of Purchases and Supplies no later than the due date and time to be deemed responsive. The check must also be scanned and uploaded with the e-bid. Company checks and personal checks will NOT be accepted. Checks shall be made payable to "City of Toledo."
