The existing bridge is a concrete double box culvert, which has reached the end of its useful life. This culvert will be removed and replaced with a new cast in place12’x13’doubleboxculvert.Thisprojectislocatedalong2nd Avenue South approximately 2.5 miles west of Mill Creek Parkway. See the project location map on Page 1 of the attached plans. PROJECT SCOPE The scope of services to be performed by the Consultant shall be completed in accordance with generally accepted standards of practice, the Iowa DOT Design Standards and Specifications and shall include the services and supplies to advance the project into construction. The scope of services shall include, but is not limited to, the following tasks: 1. Provide a review of General Contractors, materials, and submittals 2. Providing project construction inspection in accordance with Iowa DOT requirements 3. Providing testing & reporting of cast in place structural concrete and concrete pavement per Iowa DOT requirements 4. Preparing and reviewing pay estimates and change orders 5.
