SOURCES SOUGHT STATEMENT OF WORK BACKGROUND: The Albuquerque Area Indian Health Service, Acoma-Canoncito-Laguna Indian Health Center (ACL IHC), has a need to remove an existing roll down door and replace with a new security roll down door for the ACL IHC Health Information Management (HIM) Department which is located at 80B Veterans Blvd., Acoma, New Mexico 87034. The Contractor shall remove the existing door and install the new door. Upon removal of the existing roll down door, the Maintenance Department will frame and enclose a portion of the counter opening to accommodate the new downsized roll down door. The Contractor shall return and install a new security roll down door at the counter when the framework is complete. Work to be performed by the Contractor, but not limited to, is the following: 1. Remove the existing roll down door and associated hardware and properly dispose of door and all material. 2. Thoroughly clean counter and channels for installation of new door. 3. Install all required hardware and new roll down door per manufacturer's recommendations. 4. Door shall be as follows: a. Steel b. 22 Gauge c. Dimensions - 2'-3.25" x 3'-8.75" d. Between jamb mount e. 1 1/2 hour rated fire door f. Motor controlled 5. Perform test to ensure door operates per Manufacturer's requirements. 6. Provide warranty information and operator's manuals. PURPOSE: Currently the Indian Health Service, Albuquerque Area does procure services and supplies from Small Business Indian Firms pursuant to the Buy Indian Act 100% of the time. STATEMENT OF OBJECTIVES: 1. Locate eligible and qualified Small Business Indian Firms to support IHS procurement needs under NAICS code: N099 - Installation of Equipment- Miscellaneous / 238290 - Other Building Equipment Contractors. 2. Utilize the information received with future acquisition planning efforts and potential Buy Indian set aside. 3. Possibly create one Firm-Fixed Price Contract amongst eligible and qualified Indian Small Business Economic Enterprise (ISBEE). HOW TO RESPOND: This is a sources sought notice under the authority of the Buy Indian Act. The Indian Health Service (IHS) is attempting to locate qualified and eligible ISBEE Firms, "Native American Indian Owned and Operated businesses," that can provide services/supplies to the Indian Health Service. This is a request for information only. Responses to this notice are not considered offers and cannot be accepted by the Government to form a binding contract. Any information submitted by respondents to this notice is strictly voluntary with no obligation or incurred cost by the government. Provide With Response: 1. E-mail response.
