This Request for Proposal is issued by UHN for the purpose of soliciting Proposals from interested Proponents for the provision of Emergency Remediation Services to UHN and (at their option) other Purchasers. Note: This RFP and resulting Emergency Remediation Contractors will replace the current Plexxus list of Emergency Remediation Contractors. The Work is to be performed from the contract award date (anticipated to be December 2024) for a period of 3 years. The description of any options as follows: 2, 1-year periods each The deliverables are listed as follows: The program delivery and scope of work includes, but is not limited to: Step 1: Receive call/email from FM-PRO Facilities Operations in regards to water/mould damage o Project Manager from Contractor to respond on site within 1 hour for an initial assessment, with professional attire. o Contractor to confirm the immediate requirements with FM-PRO Facilities Operations on-site contact and dispatches the required resources within 12-48 hours to start remediation work. Step 2: Dispatch outlined crew of remediation technicians to build soft hoarding to isolate specific areas, set up drying fans, set up dehumidifiers within 4 hours after initial assessment by the Contractor or as advised by FM-PRO Facilities Operations. Step 3: Assess each situation confirming with FM PRO Redevelopment Projects and/or Small Projects the identified requirements: o Prepare an Infection Control Plan as per CSA Z317.13-17 that includes but not limited to: hoarding plan, materials to be used, ante room(s), venting location and proposed routing, metal doors. o Reporting requirements and protocols (communication). o Risk identification, factors, and assessment. o Draft work schedule (to be prepared within 48 hours of site assessment). Work schedule to be updated as required due to nature of work. o Draft phasing plan (to be prepared in coordination with UHN) submit for IPAC review within 48 hours. Phasing plan to be updated as required due to nature of work. o Prepare draft budget pricing (identify long lead items, regular hour labour versus after-hours fees for all required trades) within 48 hours of receiving 3rd Party Environmental Consultant scope of work. Step 4: Receive direction from 3rd party Environmental Consultant via a written report and coordination with UHN IPAC. Complete the work in accordance with the protocols provided, in conjunction with the CSA and UHN standards and policies. o Adhere to all UHN Safety Services, FM-PRO, & IPAC rules and guidelines. o Construct hard hoarding to the satisfaction of UHN IPAC's approval prior to starting remedial work. o Negative air must always be vented through exterior window when available. Contractor responsible for safely removing window glass. If Facilities does not have suitably sized window insert panel for use, Contractor responsible for supplying and installing a new metal insert window panel comprising of metal outside faces complete with rigid insulation core, weather deflection shrouds and bird screens. All metal insert window panels to be handed over to UHN Facilities upon project completion. o Repair, remove, dispose, and or replace all materials as required, including but not limited to, walls, studs, drywall, ceiling, millwork, flooring, base, doors, fixtures, furniture, etc. o Repair, remove, dispose and/or replace all electrical and mechanical items as identified. o Verify with FM-PRO Redevelopment Projects and/or Small Projects for any items to be returned to owner for reuse. o Remove, dispose, protect and or relocate all FFE as required and as directed by FM-PRO Redevelopment Projects and/or Small Projects. o Complete investigative report identifying root cause of mould, leak, etc. Prepare and present remedy scope of work, costs, and time schedule for FM-PRO review. Step 5: Closeout o Ensure all air sampling testing is as per 3rd party testing report and protocols. o Supply all required back-up documentation as required to satisfy Insurer requests as applicable, at no additional cost. o Prior to removal of hard hoarding, review remediated area with UHN IPAC. o Upon completion of work, do a final cleaning, leaving the premises ready for use. o Holdback if there are progress draws in place.
