On-call locksmith services and keys for the City of Chelsea Department of Public Works SCOPE OF WORK Description of Services: Provide labor and materials to troubleshoot and repair locks on-site for City of Chelsea public buildings and structures, including interior and exterior doors and gates. The city buildings contain a variety of commonly used model of tumbler locks. No electronic keypads are included in this scope of work. Provide duplicate keys as request for repaired or replacement locks. This work is subject to Massachusetts Prevailing Wage. Forms All forms listed below must be completed including: Quote submission, Statement of Corporate Authority, Certificate of Non-Collusion, Tax Compliance Certification, Wage Theft Certification, Living Wage Certification, {OSHA Compliance - if Construction related}, Conflict of Interest Law Certification and Requisitions and Purchase Orders - Terms & Conditions. Rule for Award This contract is procured under M.G.L. {Chapter 30B, Chapter 30 39M, Chapter 149}. The contract will be awarded to the lowest responsive and responsible quote(s). All prices must remain constant for the duration of the contract. The City herein declares its express purpose not to award the contract to any proposer unable to furnish evidence, satisfactory to the City, that it has sufficient ability, experience, and capital to execute and complete the work in accordance with the contract. If requested, any proposer may be required to demonstrate financial stability satisfactory to the City. Length of Contract This contract is anticipated to be executed on or about 11/1/24. The City reserves the right to change, delay, cancel, or expedite the contract execution date. The selected proposer is required to furnish all certificates of insurances required under the contract, in a form acceptable to the City prior to the execution date. The contract will be for a period commencing 11/1/24 and expiring on 6/30/25. Taxes Purchases made by the City are exempt from the payment of Federal excise tax and the payment of Commonwealth of Massachusetts sales tax and any such taxes must not be included in the quote prices. The City will provide the bidder awarded a contract under this bid a copy of the City's tax exemption certificate, upon request. Insurance The successful bidder in addition to any insurance required by State or Local Law, shall maintain in force during the term of any contract(s) issued as a result of this Invitation for Bids, the following insurance issued by an insurance company licensed to do business in the Commonwealth of Massachusetts. Failure to provide or maintain such insurance shall be grounds to reject a bid or execute a contract. a. Public Commercial Liability coverage in the amount of $1,000,000.00 per occurrence and $2,000.000.00 aggregate. Automobile Liability Insurance coverage in the amount of $500,000.00 per person, $1,000,000.00 per occurrence. Property damage Insurance in the amount of $250,000.00. b. Worker's Compensation Insurance in the amounts required by Massachusetts Law. c. Evidence of such insurance must name the City of Chelsea as the named insured as well as the successful bidder. d. An Insurance Certificate giving evidence of the insurance must be delivered to the City of Chelsea within 10 days by the successful bidder receiving the award of this Invitation for Bids. Prevailing/Minimum Wages {Remove if Not Applicable} Prevailing/Minimum Wage Rates as determined by the Commissioner of the Division of Occupational Safety of the Executive Office of Labor and Workforce Development under the provisions of Massachusetts General Laws Chapter 149, Section 26 to 27D, as amended, apply to this project. It is the responsibility of the Bidder, before the bid opening, to request, if necessary, any additional information on Prevailing/Minimum Wage Rates for those trades' people who may be employed for the proposed work under this contract. Funding & Fiscal Year Appropriation Appropriations for expenditures by the City of Chelsea and authorizations to spend for a particular purpose are ordinarily made on a fiscal year basis. The fiscal year for the City of Chelsea, MA begins on July 1st and ends June 30th of the following year. The obligations of the City of Chelsea under any contract resulting from this Invitation for Bids for any subsequent fiscal year following the fiscal year in which the initial contract is awarded is subject to the appropriations to the City of Chelsea of funds sufficient to discharge its obligations, which accrue in such subsequent fiscal year, and to the authorization to spend such funds for the purposes of this Invitation for Bids. In the absence of such appropriation or authorization, any contract resulting from this Invitation for Bids shall be terminated immediately without liability for damages, penalties or other charges arising from early termination. Expenditures for contracted services, which will extend beyond a single fiscal year shall not exceed in any fiscal year the amount appropriated and authorized for the said fiscal year. Invoicing All invoices are to be sent to {City of Chelsea, Public Works, 500 Broadway, Room 310, Chelsea MA 02150}. General Notices The bidder shall be expected to comply with all applicable federal and state laws in the performance of services.
