The District is soliciting proposals from Class A or C-57 Contractors to be placed on its on-call list of approved contractors to augment the District's capacity for as needed repair and rehabilitation of its groundwater production wells. It is anticipated that up to four (4) contractors will be awarded an On-Call Non-Professional Services Agreement (NPSA) to provide non-exclusive on-call well repair and rehabilitation services and work on a Task Order basis. The term of the NPSA will be three (3) years, with the District's option to extend the NPSA for up to three (3) additional years. The Not to Exceed (NTE) amount for the sum total of all NPSAs awarded will be $2,000,000 for the initial three (3) year period, with the option to increase the NTE. A time extension to the initial three (3) years and/or an increase in the NTE, if exercised by the District, will be in writing and signed by both the District and the Contractor. The District does not guarantee a minimum number and dollar amount of Task Orders that may be assigned to each Contractor under an NPSA, nor does the District guarantee that each Contractor will be assigned an equal number and dollar amount of Task Orders with respect to other Contractors awarded an NPSA. The information requested below will be used to evaluate the Consultant's Proposal. The Proposal shall include the following information: a. The proposal is limited to 10 pages, exclusive of cover letter, preprinted materials, and Hourly Cost Plus Rate sheets. b. Cover Letter: A letter signed by a principal or authorized representative who can make legally binding commitments for the Proposer. Include statements regarding ability to comply with prevailing wages, certificates of liability insurance and other insurance requirements, and the District's standard agreement. c. Statement of Qualifications: Include a brief description of services to be provided by Proposer, qualifications of key staff, and the name and contact information for the Proposer's project contact with the District. Include Contractor's license classification(s) and number(s). Include a brief description of services to be provided by Sub-contractor(s) (if applicable). Include Sub-contractor's license classification(s) and number(s). d. Project Experience: Proposer shall submit a brief description of the well repair and rehabilitation experience performed in the last five years. Proposer shall also submit a brief description of the Sub-contractor(s)' work experience. Include prior experience of work with the District, if applicable. e. References: The Proposer shall submit contact information for a minimum of three (3) agencies, past and present, for whom you have provided similar work in the last five years and include project information if not included in the Project Experience. f. Hourly Cost Plus Rate Sheets: Provide detailed Hourly Cost Plus Rates (ATTACHMENT A) for each of the three (3) years included in the initial agreement term.
