Request for Information 12.6.2024 & Amended Clauses and Provisions The Whiteriver Indian Health Service Hospital requires a contracted project for the replacement of five (5) 6-inch water system valves. The contractor shall procure and install five (5) new Hydra-Stop, Insta-Valve 250 Patriot 6-inch valves. IAW attached Scope of Work (SOW). The period of performance of this contract is 60 calendar days after award Question Deadline 12/12/2024 at 1:00 PM MT All work shall be performed at the Whiteriver Service Unit (WRSU) Medicine City housing complex located at adjacent to 200 West Hospital Drive, Whiteriver, Arizona 85941. The Whiteriver Indian Health Service Hospital requires a contracted project for the replacement of five (5) 6-inch water system valves. The scope of work detailed below outlines the required project outcomes. The Contractor shall be responsible for all necessary work to achieve required outcomes including labor, equipment, tools, personal protective equipment (PPE), incidental design, travel, per diem, tribal fees, and other related expenses required to complete the project. The contractor shall procure and install five (5) new Hydra-Stop, Insta-Valve 250 Patriot 6- inch valves. All work shall be completed by the contractor including excavating, valve concrete support installation (as needed), valve installation, riser installation, and backfilling.
