General Requirements - Certified Payroll - Insurance Required - Prevailing Wage Required cope of Work: 1. GYM #1 (HS Gym - 50 Iroquois Drive, Salamanca, NY 14779) a. Complete preparation of gym floor (75' x 95') using 3M SPP pads at a rate of 250 quare feet per side of pad. b. Complete vacuum and tacked/preparation of the gym floor. c. Apply one coat of Hillyard Trophy H2O water-based finish. d. Warranty for one year after completion. 2. GYM #2 (Auxiliary Gym - 50 Iroquois Drive, Salamanca, NY 14779) a. Complete preparation of gym floor and hallway (48' x 79' and 8'x40') using 3M SPP pads at a rate of 250 square feet per side of pad. b. Complete vacuum and tacked/preparation of the gym floor. c. Apply one coat of Hillyard Trophy H2O water-based finish. d. Warranty for one year after completion. 3. Special Considerations/Conditions 1. All product, equipment and labor will be provided by contractor. a. Vendors will note that prevailing wage rates, as determined by the NYS Bureau of Public Works Labor Department, may be applicable for all phases of the work on this bid. Certified payrolls must be submitted along with invoices. Bid participants will be responsible for obtaining prevailing wage schedule and PRC case # information from the NYS Dept of Labor. 2. Salamanca City Central School agrees to: a. provide an electrician to hook up and unhook electric as needed. b. provide a dumpster for debris. c. turn on ventilation system 1 hour after coat of finish is applied. 3. The District and selected contractor will work on a mutually agreeable timeframe. Once started, project should be finished within 10 days. *The Board of Education reserves the right to reject all bids.
