This Request for Supplier Qualifications (RFSQ) is issued by MMC on behalf of Potential Purchasers for the purpose of soliciting Proposals from interested Proponents for the creation of a regional Prequalified Supplier List (PSL) in the category of Elevator & Escalator Maintenance Services. It is the intention that under this RFSQ, Prequalified Proponents will enter into an Agreement, in joining the PSL as an outcome of passing the minimum score threshold in the First Stage Process. The Term of the Agreement will be five (5) years with an option in favour of MMC to extend the Term on the same terms and conditions for two (2) additional periods of two (2) years each. A Second Stage Process (RFQ) will subsequently be conducted on behalf of Potential Purchasers that will determine the winning supplier on a hospital-by-hospital basis. The contract award will be added to the winning Suppliers Agreement using the Purchaser Participation Schedule. The PSL will be divided into 6 regions of Ontario as follows: 1. Eastern Ontario 2. Toronto 3. Central Ontario 4. Southwestern Ontario 5. Northeastern Ontario 6. Northwestern Ontario Proponents are to indicate the regions they would like to prequalify for as part of this RFSQ. Geography details of the regions are found in the Map of Service Regions Schedule.
