2025 Lawn Maintenance Program Question Deadline 01/02/2025 at 4:00 PM ET Questions must be submitted thru the Q & A tab of this bid Online Yes Bid Bond $250.00 Preferences MBE - Minority-Owned Business Enterprise WBE - Woman-Owned Business Enterprise The following items will be required upon awarding the contract: Performance Bond and PLA, if applicable. As noted above, City forms must be filled out and attached to the bid. Project Paperwork includes the following: Acknowledgment and Acceptance of Conditions Contract Specifications; Toledo Municipal Income Tax Compliance; Public Utilities Compliance; MBE Good Faith and Goal Commitment; EEO Form; Primary and Subcontractor Forms and Debarment and Suspension Certificate; Best Bid Criteria. Other required forms include those noted in the Response Type Section and also noted in the Other Details Section: Signed Contract by Vendor CEO/President; Child Support Affidavit; Insurance Certificate listing the city of Toledo as an additional insurer; and Current Worker's Compensation Certificate. The bid guaranty shall be in the form of a cashier's check/official check on a solvent bank or a bid bond. If using a check as the bid guaranty, the check must be received by the Division of Purchases and Supplies no later than the due date and time to be deemed responsive. The check must also be scanned and uploaded with the e-bid. Company checks and personal checks will NOT be accepted. Checks shall be made payable to "City of Toledo." In order to be responsive, all bids must have the following with submittal: o Project Paperwork* o Line Items - will auto-populate with e-bid o Bid Guaranty (bid bond or certified check / cashier's check on a solvent bank) - see instructions below o Signed Contract by the Vendor* o Child Support Affidavit* o Bureau of Worker's Compensation Certificate or Waiver and Release of Liability Form o Insurance Certificate, naming the City of Toledo as an additional insured / Certificate Holder - requirements available on the "Documents" tab of PlanetBids o Equipment Inventory and Staff List o Taxation Certificate of Compliance Scope of Services The City of Toledo Division of Urban Beautification is seeking multiple vendors to provide all labor, materials, equipment, and service on a schedule as directed, necessary to perform grass and all vegetation cutting services for several city parks, right-of-ways, and neighborhood parcels. If your company is using a bid bond, scan and upload with bid; the original does not need to be received by the City of Toledo. If your company does not provide a bid guaranty, your bid submittal will be deemed non-responsive. (TMC 187.09)
