Renovation of a mixed-use development in Red Mesa, Arizona. Completed plans call for the renovation of a medical facility.

Navajo Area Indian Health Service, Shiprock Service Unit, Four Corner Regional Healthcare Center, needs a contractor to provide construction services to replace the water damage tiles areas with new material located in the Wellness Area - Ladies Locker Room. Random leaks were occuring in women's locker room and the deperatment found that the current grout in the shower were not sealed thus causing leaks to the area. Contractor shall provide all labor, materials, equipment, transportation, supervision, and all other necessary items for a complete and useable product in full comliance with the Scope of Work, plans, drawings, and specification as defined in the solicitation. To submit a quote, review the enclosed documents. this solicitation is considered construction and is conducted as a 100% ISBEE Indian Small Business Set-aside. Estimated range: $56,000.00; Davis Bacon Rates Apply - Attached; NAICS Code: 327120; Small Business Size Standard: 750. The contractor, at no additonal expense to the Government, may coordinate with Point of Contact: Alvin John, Maintenance Mechanic Supervisor, 928-656-5044 to request a pre-bid visit. **Refer to attached solicitation IHS1504498. See Section K Proposal Instruction. Site Visit An organized site visit has been scheduled forDate and Time to be designated by the Contracting Officer. (c) Participants will meet atFour Corners Regional Healthcare Center, Red Mesa AZ 86514 Introduction: This statement of work describes the requirements for an outside contractor to replace the Turquoise Wellness Ladies Locker Room tile repairs at the Four Corners Regional Healthcare Center. 1.1 Background: Biomedical Office has experience random water leaks near the shower locker room wall. During our investigation of the water leak EVS staff has informed of random water leaks to corridor near locker room. Facility maintenance went ahead and open the wall on biomedical side that share same wall as showers. There are no obvious was leaks from the internal piping. But as we turned the showers on and soon we noticed water coming from bottom of shower. It was determined the grout in the shower grout area was not sealed and was causing the water leaks to floor. The contractor shall remove all water damaged materials and dispose of materials properly. The contractor shall remove and reinstall all equipment in locker room such as benches, lockers, shower fixtures and etc. The contractor shall install proper water sealants in tiled and applicable wall areas; and tile locker room. The Indian Health Services (IHS) is an agency within the U.S. Department of Health and Human Services and the mission is to raise the physical, mental, social, and spiritual health of American Indians and Alaska Natives to the highest level. 1.2 Place of Performance: Worksite is at the Four Corners Regional Healthcare Center- Indian Health Service located on Junction U.S. Hwy 160 & Navajo Route 35 - Red Mesa, Arizona 1.3 Scope: The Contractor shall replace the water damage are in the Turquoise Wellness Ladies Locker Room tiled areas. Any additional necessary parts, the contractor will coordinate with the Contract Officer regarding this matter. 1.4 Special Conditions: Four Corners Regional Healthcare Center is open daily Monday Friday from 8:00AM to 4:30PM. The Contractor shall not obstruct the access and/or egress of exits. 2. Performance Period: The contractor shall accomplish all installation and training within ninety (90) consecutive calendar days after official "Notice to Proceed" is granted by the Contracting Officer (CO). 3. Government Furnished Property: 3.1 The Government will NOT provide tools, labor, materials, supplies, transportation, equipment, supervision, personal protective equipment, other items to perform required services. 3.2 52 236-14 Available and Use of Utility Services: The government shall make all reasonable required utilities available to the Contractor from exiting outlets. The Contractor shall carefully conserve utilities furnished without charge. 4. Contractor's Furnished Property: 4.1 The Contractor shall provide their own tools, labor, materials, supplies, transportation, equipment, supervision, personal protective equipment no smoking including vaping on facility grounds or other items to perform required services. 4.2 Provide a written recommended care and maintenance guideline. 4.3 Provide a written warranty on materials and labor on finished product. 4.4 Deliverable: The Contractor will coordinate with the Facility Manager Tanya Noble Sudjadi at 928-656-5045 or call the switchboard operator at 928-656-5000. 5. Contractor Requirements: 5.1 Contractor shall furnish all materials, equipment, tools, and any other items to complete the job. 5.2 Provide all personal protective equipment by the contractor's personnel for safe implementation of their responsibilities. 5.3 No smoking, No tobacco use, no vaping on government grounds. 6. Administrative Requirements: 6.1 Contractor shall report to security office for temporary badges. 6.2 Contractor shall sign in at the Facility Management Department. 6.3 Contractor shall stay in contact and coordinate with the CO before and during the period of performance. 6.4 Contractor shall perform work during business hours, 8:00AM to 4:30pm. 6.5 Contractor is responsible for any damages to Government Property caused by the Contractor's employees and/or equipment. 7. Quality Assurance: 7.1 Follow the Safety Data Sheet (SDS). 7.2 Standards as required by Federal Statues for substances, materials or products must be provided for all project. 7.3 Defective materials/latent defects caused by the manufacturer must be replaced at no charge.
