KDOT District One Headquarters - Bid Package #2 - Sanitary Sewer Improvement Project Work included: Trade Contract 31A Scope of work including, but not limited to: General Requirements, Complete, single source, turnkey installation for the Sanitary Sewer Improvement Project No. 401122.00 including but not limited to excavation, tree and vegetation removal as required, road crossings, traffic control, backfill and compaction, concrete and asphalt patching, surveying, piping, precast structures, erosion control, and final seeding as required for a complete project. Attention to the following bid requirements for a bid to be determined responsive: - Complete, sign and include Attachments A, B, C, D, & E found at the end of this bid package document along with your bid form and bid bond (if required) with your bid submission. - Surety Bid Bond for 5% of Base Bid will be required for all bids over $500,000. - Performance, payment, and maintenance bonds will be required by the lowest responsible bidder for any subcontractor whose bid exceeds $100,000. These costs are to be included in your bid amount per the bid form. All questions shall be furnished in writing via email to: Kenny Hartter: khartter@crossland.com or call him on his (cell) (785) 230-6107 **Total Project Estimate Value is Listed 1) Scope of work includes a turnkey installation but is not limited to the construction documents sheets C600- C612 Titled SANITARY SEWER IMPROVEMENT PROJECT NO. 401122.00, all complete. 2) Include all traffic control as required. 3) Include new, permanent signage as indicated in the construction documents. 4) Include sweeping of roads and streets due to track out. 5) Include protecting existing trees and vegetation to remain. 6) Removing trees and other vegetation. 7) Clearing & Grubbing as required. 8) Surveying, Staking and Layout to be included in this trade contractors' scope of work for all work associated with this bid package. 9) Removal, replacement and proper compaction of all road and entrance crossings. Replacement shall consist of similar materials at the time of removal of said road and entrance crossings. Including any striping that may be required. 10) All piping, connections, precast structures, rip-rap, tap fees, testing, etc. that are required for a complete installation. 11) All pipe bedding, flowable fill, etc. for a properly backfilled and compacted systems. 12) Includes establishing all SWPPP requirements (including silt fence, rock ditch checks, sediment basins, drain waddles, curb inlet protection, construction entrances, etc.) per the Project Documents, KDOT requirements, maintaining, and removal once vegetation is established. 13) Include a complete and finished sanitary sewer installation as indicated on the project documents. 14) Include all utility work and earth moving, trenching, backfilling as indicated, but not limited to sheets C600, C601, C602, C603, C604, C605, C606, C607, C608, C609, C610, C611, and C612, etc. 15) Include potholing and locating of existing utilities as required. 16) Includes all fees associated with utility work including, but not limited to impact fees, tap fees, etc. 17) Include installation by licensed plumbers as required by the AHJ. 18) Include all seeding / mulching required to return disturbed areas to like condition prior to construction. 19) Bid bond equal to 5% of bid amount. 20) Payment and performance bonds. 21) Sales Tax
