In Addition to Submitting the Complete Proposal on a Flash Drive, Please Submit One (1) Clearly Marked Unbound Original (3-hole Punched Preferred) and Five (5) Copies of the Proposal in a Sealed Envelope or Box Addressed to the City Clerk, City of Daytona Beach Shores, 2990 S. Atlantic Avenue, Daytona Beach Shores, Fl 32118, and Plainly Marked on the Outside " Engineering Services for Sewage Collection and Resiliency Upgrades" the City is Not Responsible for the U.s. Mail or Private Carriers or Messengers Regarding Delivery to the City by the Specified Time. The City Reserves the Right to Reject Any or All Submittals and to Waive Any or All Non-substantial Irregularities in Items Received Whenever Such Rejection is in the Best Interest of the City, or to Accept the Proposal Which Best Serves the Interest of the City. In Accordance With Florida Statutes 287.133(2)(a), Any Person or Affiliate Who Has Been Placed on the Convicted Vendor List Following a Conviction for a Public Entity Crime May Not Submit a Bid on a Contract to Provide Any Goods or Services to a Public Entity, May Not Be Awarded or Perform Work as a Contractor, Supplier, Subcontractor, or Consultant Under a Contract With Any Public Entity, and May Not Transact Business With Any Public Entity in Excess of the Threshold Amount Provided in Section 287.017 for Category Two for a Period of 36 Months From the Date of Being Placed on the Convicted Vendor List.
