Michigan Department of Transportation (MDOT), Oakland Transportation Service Center is seeking a Prequalified Maintenance Contractor to perform concrete curb and gutter repairs, including Detail M opening repairs, along eastbound and westbound Highland Road (M-59) from Bogie Lake Road to Pontiac Lake Road. The Contract will require removal and disposal of existing concrete curb and gutter, installation of concrete curb and gutter including Detail M openings, catch basin adjustment, providing traffic control, and other work in accordance with these Contract Provisions, Attachments, and referenced Documents (the "Contract Activities"). 1. REQUIREMENTS The Contractor (or subcontractor) must be currently listed as a MDOT Prequalified Contractor, in in category J - Concrete, C, C&G and category Ea - Grading, Drainage Structures & Agg. Cons. 1.1. General Requirements A. The Contractor shall provide all labor, materials/supplies, equipment, tools, supervision, and any incidental services or items required to perform the Contract Activities. B. The Contractor must notify the MDOT Program Manager 7 Days in advance prior to the start of all work operations. This will allow time for Program Manager to mark all locations prior to start of work; and for applicable third-party quality assurance (QA) technicians to be notified of any upcoming construction activities. C. At no point shall work begin without a representative from MDOT on site.
