The bid shall be submitted on the specified Contact Sheet, hereto attached as "Exhibit A" and Bid Pricing Form, hereto attached as "Exhibit A-1". The person signing the Bid Response Form shall have the authority to bind the proposer to the Bid. All information on the Bid form shall be provided, or the Bid may not be accepted. In accordance with Item 16, General Terms and Conditions, please provide proof the Contractor/Vendor is not listed in the Excluded Parties List System, a federal suspension and debarment listing. SCOPE OF WORK The City of Fernandina Beach, Florida (City) is requesting Guaranteed Maximum Price proposals from qualified contractors to provide construction services for the installation of approximately 1,300 linear feet of new water main and 2,000 linear feet of sewer force main that will provide service to a developed area of Friendly and Russell Roads on Amelia Island, Florida. All roadways are two lane asphalt pavement, and rights of way are owned by Nassau County. The 8" water main on Friendly Road will connect to an existing 8" water main on Bonnieview Road. The 4" force main will connect to an existing gravity stub at Bonnieview and Russell Road. The City of Fernandina Beach will coordinate approval for this work with Nassau County. The utility installation along the right of way may be installed by trench or directional bore. All roadway crossings shall be repaired with flowable fill. Final asphalt repair will be completed by the City. The successful bidder is to perform all installation, testing and restoration necessary to obtain final clearance from FDEP and Nassau County. The successful contractor will provide all materials, equipment, labor, tools, and supervision necessary to perform the installation of the new force main in accordance with the Construction documents prepared by the City. The contractor and any subcontractors shall be properly licensed as provided by the applicable federal, state, and local laws and regulations. SAFETY The Contractor shall provide all necessary barricades and signage and take necessary precautions to protect buildings, general public and Contractor personnel. The Contractor shall at all times guard against damage or loss of City property or the general public and shall be held responsible for replacing or repairing any loss or damage to the satisfaction of The City of Fernandina Beach. AWARD Award recommendation shall be made to the most responsive and responsible bidder offering the best value. The City reserves the right, based upon its deliberations and in its opinion, to accept or reject any or all proposals. The City also reserves the right to waive minor irregularities or variations to the specifications and in the bidding process. Successful Bidder is required to have a Business License in the city where their home office is located. If Bidder's business office is located in the City of Fernandina Beach a business tax license is required.
