Each proposal bid must be accompanied by a bid bond or certified check for not less than ten percent (10%) of the amount of the bid, of which, the amount of the bond or certified check need not exceed $20,000, made payable to the TOWNSHIP OF GREENWICH. All blank spaces in the proposal, except as otherwise noted, must be filled in; and no change shall be made in the phraseology of the proposals or of the item mentioned. The proposed work includes the following major items: o Provision and connection of three (3) hydropneumatic tanks o Disinfection, bacteria and total coliform testing o Removal and disconnection of tanks All information that may be requested of the bidder in these specifications shall be included with this proposal. All erasures, interpolations, or other physical changes on the bid form shall be initialed or signed by the bidder. Bidders are required to comply with the requirements of N.J.S.A. 10:5-31 et seq. and N.J.A.C. 17:27 (Affirmative Action), P.L. 1963. C150 (New Jersey Prevailing Wage Act), and Americans with Disabilities Act of 1990 (42 U.S.C. 12101, et. seq.). The contractor is notified that he must comply with P.L. 1977, c. 33, and submit a Disclosure Statement listing Stockholders with his bid. The contractor is further notified that he must comply with P.L. 1999 c. 238 Public Works Contractor Registration Act and he and any subcontractors must be registered in accordance with this act. The contractor is also further notified that he must comply with P.L. 2004 c. 57 and submit proof of a New Jersey Business Registration Certificate and submit proof of business registration for any named subcontractors in accordance with the act. This contract is subject to the provisions of the New Jersey Public Contracts Law, N.J.S.A. 40A:11-1, et seq.
