Such Proposals as Herein Concerned Shall Be for the Following as Described: the Board of County Commissioners is Requesting Competitive Sealed Proposals for the Purchase, Potential Temporary Climate Controlled Storage, Professional Installation and Warranty of Office Style Cubicle and Loose Furnishings for Wood County Department of Job and Family Services, 1928 East Gypsy Lane Road, Bowling Green , Ohio 43402. A Bid Bond, Certified Check, Cashiers Check, or Money Order on a Solvent Bank or Savings and Loan Association, in Favor of the Wood County Commissioners, in the Amount of Five Percent (5%) of the Proposal Shall Accompany Each Proposal Submitted. All Proposals Shall Be Sealed and Plainly Marked: Jfs Cubicle Remodel Project. The Successful Offeror Will Be Required to Sign: (1) an Affidavit of Contractor or Supplier of Non-delinquency of Personal Property Taxes in Accordance With Orc 5719.042; (2) an Non-collusion Affidavit; (3) an Affidavit Regarding Child Support, Pursuant to Bcc Resolution 92-2041 and (4) a Bid Bond, (as Noted Above). The Board of County Commissioners Reserves the Right to Waive Any Irregularities in the Form of the Proposals. In No Case Will an Award Be Made Until All Necessary Investigations Are Made as to the Qualifications of the Offeror to Whom It is Proposed to Award the Contract. The Board of County Commissioners May Reject Any or All of the Proposals. Purchase, potential temporary climate controlled storage, professional installation and warranty of office style cubicles and loose furnishings for WCDJFS. The Scope of Work is described in Attachment A. Please refer to Attachment B for specific cubicle and loose furnishing specifications. For general reference, Attachment B indicates the need for Phase 1 and Phase 2. Phase 1 includes 37 fully furnished cubicles. Phase 2 includes 55 fully furnished cubicles as well as 4 additional cubicle style offices with doors. Also included are additional cubical panels to be added onto existing cubicles. Please refer to Attachment C for design and mapping information. The County, on behalf of the Wood County Department of Job and Family Services, will award one contract. All RFPs are contingent upon, among other things, availability of funds.
