Cherokee Nation Businesses, LLC, a tribal corporation of the Cherokee Nation, is seeking bids to install new flooring and perform site grading at our Employee Health Clinic located North of the Hard Rock Casino in Catoosa, OK. The Owner intends that if an acceptable proposal is received, the selected firm will be notified in writing within 10 business days of the bid opening. Any bid received after the time set for opening of Bids, shall not be considered and will be returned. It is the intention of CNB that if an acceptable bid is received, the Contract will be awarded within 30 days of bid opening. Please note that winning bid(s) will be published. CNB reserves the right to issue one award, multiple awards, or reject all bids. All bids are subject to negotiation before award. Awards may be issued without discussion of the bid received, and bids should initially be submitted on the most favorable terms from price and technical standpoints. Any award resulting from this Bid/Proposal will be subject to CNB Terms and Conditions. Your bid should indicate the hourly ($) rate (when applicable), individual work effort, delivery schedule, or time of completion. Any item(s) with exceptions, substitutions, exclusions, additions, or qualifications must be noted and indicated on the return bid. The following items will need to be included in the Contractor's scope of work: o Temporary relocation and replacement of desks, tables, boxed items etc. necessary to perform the demo and install of new flooring. o Demolition and disposal of existing flooring, base and adhesive. o Provide necessary protection to walls, millwork, windows, doors etc. during demolition. HVAC should be protected via filters or shutdown during the demolition. o Preparation of existing slab and install of new high moisture resistant adhesive. o Installation of new LVT and rubber base - Milliken brand or comparable product, (approximately 3,700 SF -contractor to field verify) o Demo and haul-off of all concrete sidewalk sections that are not properly falling away from the building along the east and south side. o All subgrade below sidewalks must be compacted and have a minimum of 6" of rock below. o Form and place sections of removed sidewalk to properly fall away from the building. All new concrete placed needs to be minimum 3500 psi and should meet ADA guidelines for sloping. Caulking and sealant will need to be installed along the building and expansion joints. o Caulking, sealants and/or grout to be installed along the west sidewalk and side of the building to ensure there is no future water intrusion to the building slab. This also includes the area around the HVAC equipment. o A new mow strip will be installed along the north building edge with proper slope and sealants, (minimum 18" wide with 4" of rock below compacted) o Grade work will need to be adjusted around the building to ensure proper site drainage. Grading shall be clear of large rocks and prepped for sod install. o All disturbed green areas must be re-sodded with Bermuda sod.
