Your submittal must contain the following items: A completed Schedule of Prices, which shall include all, costs for labor, material. equipment, supervision, tools, insurance and other costs necessary to perform the work. Brand and type of materials to be used. A quality control plan, Project work schedule, and duration. Project needs to be completed by June 30th 2025. A signed PERA Form, your W9 and if applicable a letter stating you participate in National Joint Pricing Agreements ie. Omnia, Sourcewell or others.
