Site work and paving for a road / highway in Danielsville, Georgia. Completed plans call for site work for a road / highway; and for paving for a road / highway.

The Board of Commissioners reserves the right to reject any and all bids. All bid packages must contain a 10% bid bond, 100% Performance Bond and O.C.G.A 13-10-91 E-Verify Affidavit in order to be considered. This project consists of Patching, OGI, Overlay& Striping in Madison County. Locations will be designated by Typical section or the Engineer. In addition to the Georgia 2013 Standards Specifications and Special Provisions referenced, this work shall also be performed according to MUTCD OR the following Special Conditions. o For this project, the advance warning signs specified in Subsection 150.3.04 shall be portable signs. These signs shall be placed only during times that construction is in progress or as conditions warrant as directed by the Engineer. Signs shall be removed at all other times. Costs for all Traffic Control shall be included in the LUMP SUM TRAFFIC CONTROL price. o Any exceptions will be at the discretion of the Engineer. o Contractor and/or Subcontractor shall be responsible for all layouts. The costs for layout shall be included in the overall bid price. o Prior to commencement of work, Contractor shall provide to Madison County or (Representative) for acceptance an asphalt job mix formula. o All required testing shall be done by the Contractor and/or Subcontractor. This testing shall be performed by an independent laboratory. Test samples shall be obtained from the distributor tank, nurse tank, holding tank, and/or delivery tank as directed by the Engineer. Samples may be required from all on site tanks. Final payment will not be made until all tests results have been provided to the Madison County or (Representative). The costs of all testing shall be included in the overall bid price. o Any Loop Detectors that are damaged due to pavement preservation operations shall be replaced as per GDOT detail at locations designated by the Engineer, utilizing materials that are listed on the GDOT Qualified Products List (QPL). Testing of the loops shall be required. All necessary documentation regarding the loop detectors shall accompany the invoice before payment will be made. All costs associated with this work shall be included in the overall bid price. o Prior to placement of any surfaces, existing shoulders shall be bladed no lower in elevation than the existing pavement or shoulder. All costs associated with this work shall be included in the overall bid price. o All existing RPMs shall be removed prior to Resurfacing operations. All costs associated with this work shall be included in the Lump sum traffic control price. o o The existing 5" centerline and edge line pavement markings shall be replaced as 5" paint pavement markings for the full length of Resurfacing / Pavement preservation operations. Patching and widening operations, replace striping that is removed or damaged by operation. Temporary centerline and edge lines shall be replaced according to Special Provision Section 150. Final centerline and edge lines shall be according to the Standard Specification Section 652. RPM's (Including any Type 10's) shall be replaced for the full length of pavement preservation operations. Two RPM's will be placed on centerline at 80 foot intervals on all final surfaces. Thermoplastic pavement markings damaged by paving operations shall be replaced in kind. Thermoplastic shall be replaced according to Standard Specification Section 653. Thermo includes stop bars, arrows/words, crosswalks, hatching, R/R symbols, etc. The contractor is responsible for visiting project site to determine the amount/type of pavement markings needed. All costs for pavement markings and RPM's (all types) shall be paid for by STRIPING PAY ITEM. o o Final striping shall not commence until 30 calendar days after completion of Resurfacing / Pavement preservation operations. o Restripe Roadway according to note above. Under no circumstances shall the total invoiced amount exceed the Bid Total shown of this agreement unless approved by county. The contractor shall continuously monitor the quantities and the costs associated with the work contained in this agreement to prevent any such situation. The contractor shall notify the Engineer in a timely manner in order to prevent any such situation from occurring. The contractor shall coordinate with Madison County or (Representative) regarding any necessary time restrictions and for inspection of the work.
