The sign in sheet is a separate entity from the resident visitor's log. When signing in, be sure to check the project name listed on the sign in sheet to ensure you are recording your attendance correctly. Failure to do so will cause out to be ineligible to submit a quote. There are no exceptions. Please write clearly for corresponding purposes and make sure you include an email address and phone number where you can be reached. Late arrivers at the site will not be allowed to sign in, participate, nor submit a quote for this project. If you are unable to attend either day, you will not be able to schedule a site visit at different time. Contract term: Within 30 days of receiving the purchase order County(ies): Rockland Location: 638 Route 306, Suffern, NY 10901 DESCRIPTION: SCOPE OF WORK includes, but is not necessarily limited to the following: NEW YORK STATE OF OPPORTUNITY Roof/ Gutter Replacement. The vendor shall provide all labor and materials necessary to complete the work in a professional manner according to scope of work and architectural drawings if supplied. Work is to be completed within 30 days of award, unless modified below. This is a NYS Public Works project, and prevailing wages are to be paid. Refer to attached specification for the following scope of work. Site must be cleaned, and all materials secured at the end of each workday. Absolutely no scraps, especially fasteners, are to be left unsecured. Upon completion of Roof/ Gutter Replacement, before removal of equipment and scaffolding, contractor is to schedule a quality control and quality assurance inspection with Plant Superintendent. Remove existing shingle down to original roof deck. 2. Vendor will replace roof decking and fascia as needed. 3. Vendor will install white solid and vented soffits on the overhang undersides, gables areas, and rake areas with associated J-channel. 4. Vendor will wrap all facia with white aluminum trim and caulk as needed. 5. Installation of ice and water shield, 6' (ft.) on eaves and 3' (ft.) on rakes and valleys. 6. Installation of new drip edge, flashing, counterflashing, pipe boots, synthetic underlayment, starter shingles, and 30-year architectural shingles and ridge cap. 7. Vendor is responsible for all measurement. 8. All shingles to be installed with high wind nailing pattern. 9. OPWDD will chose shingle colors. 10. Vendor will install new 5" K-style white gutters. 11. Gutter Spec: High capacity 5" K-style gutters, .032 Seamless Gutters. Proper installation ofinside and outside miters will be used as required. Hidden fasteners will be used and spaced about 12" intervals. Contractor will need to comply to manufacture installation. 12. All Leaders are 2" x 3" white aluminum. Leaders will extend 5 ft. past the foundation. 13. The Vendor will install Micro-Filtration top screen protection on the gutter. 14. Vendor will complete all work as per NYS code and Industries standards.
