SUMMARY Carrollton-Farmers Branch Independent School District ("CFBISD" or "District") is requesting proposals from qualified Local service providers to provide E-Rate Maintenance of a District Owned Fiber Optic Outside Cable Plant per the attached specifications, terms and conditions FCC Form 470 # 250020272 AWARD SCHEDULE: Recommendations will be made to the school board at the April Board Meeting. Awarded vendor(s) will be notified via email at that time. This contract will be awarded to one vendor as determined to provide the best value to CFBISD. CFBISD reserves the right to negotiate with any or all respondents and accept or reject any and/or all proposals, to waive any formalities and/or irregularities and to award in the best interest of CFBISD. Length of Contract: All agreements executed as a result of the responses to this RFP shall be for a ten (10) year agreement with ten (10) successive one (1) year voluntary extensions based on the long-range needs of the District and mutual consent of both parties. The term of this contract shall not exceed twenty (20) years total including one year extensions. Vendors that do not provide term agreements as specified will be disqualified and any respective proposal will not be evaluated. MINIMUM REQUIREMENTS: 1. Respondents must submit all responses to this RFP via this online system. The District cannot accept any emailed responses. 2. All questions should be entered into this system. 3. Vendors shall not accept or process orders from any employee or representative of the District unless and until the vendor receives a valid Purchase Order from Carrollton-Farmers Branch ISD.
