This project is for the rehabilitation of Lift Station No. 12 and will include bypass pumping, demolition of existing piping, pumps, and controls, installation of new piping, new pumps, and new controls. The existing concrete wet well, valve vault, and meter vault will remain and be reused. Document note : Addendums will only be sent to, and Bids will only be accepted by, prospective Bidders that appear on the Plan holders List. Neither Owner nor Engineer will be responsible for full or partial sets of Bidding Documents, including addenda, if any, obtained from sources other than the Issuing Office. Each bid shall be accompanied by an acceptable certified check made payable to the City of Lawrenceburg or an acceptable bid bond in the amount equal to 10% of the total bid price executed by an incorporated surety company in good standing and qualified to do business in the State of Indiana and whose name appears on the current Treasury Department Circular 570. A Performance Bond and Payment Bond each in the amount of 100% of the Contract price will be required from the Contractor to whom the work is awarded. The City of Lawrenceburg Contractor Pre-Qualification Packet may be picked up at the office of the Safety Officer at City Hall during normal working hours. Each bidder shall submit the Conflict of Interest / Nepotism affidavit, the E-Verify Affidavit, and the Subcontractor List on the forms provided. Any bid not accompanied by the above required items shall be deemed to be a non-responsive bid. The Standard Questionnaire Form 96 (latest version) must be fully completed, including attachment of Contractor's Financial Statement as stipulated in Section III, and signed. Bidders shall be pre-qualified with the City at least 48 hours prior to Bid Opening. No Bidder may withdraw his proposal within a period of 60 days following the date set for the receiving of bids. The Owner reserves the right to retain any and all bids for a period of not more than 60 days and said bid shall remain in full force and effect during said time. The Owner further reserves the right to waive informalities and to award the Contract to any Bidder all to the advantage of the Lawrenceburg Utility Service Board or to reject all bids.
