Franklin MS, 1200 Outer Park Drive, Bid No 25-08 Springfield, Illinois. Project: D186 Franklin Middle School Gymnasium Floor Refinishing A. Owner's Project Number: 25-08 - Franklin Middle School, Generally the work includes: Renovations at Franklin Middle school and remodeling of the existing systems or areas as indicated. Gymnasium Floor Renovations; Bids shall include: 1. Properly completed Proposal Form in duplicate 2. Bid Deposit in the amount of 10% of Bid. 3. Not less than the prevailing hourly wage as determined by the Illinois Department of Labor pursuant to Illinois Revised Statutes, Ch. 489, Sec. 39s-1 et. seq. The Contract Documents, including Drawings and Specifications, are on file for public inspection at the Office of Director of Operations & Maintenance-Springfield Public Schools, 530 West Reynolds Street, Springfield Ill, and as follows: Central Illinois Plan Room 1620 South Fifth Street, Springfield, Illinois, 62703 (217) 679-1077 (plans@ciplanroom.com) All Bidders are advised that all work of the contract shall be subject to the provisions of the "Illinois Prevailing Wage Act". Prevailing wages have been determined and are on file at the Office of the State of Illinois Department of Labor, telephone (217) 782-6206. All bidders shall submit and enclose with their bid a notarized certification that the contractor is not barred from bidding as a result of either Section 33 E-3 or 33 E-4 of Article 33E, Public Act 85-+1295 (Senate Bill 2002) effective January 1, 1989. Certification in located in the Proposal Form, Section 00 4000 of this Project Manual. Bidders will be required to provide Bid security in the form of a Bid Bond of a sum no less than 10 percent of the Bid Amount payable to the Springfield School District #186. Failure of bidders to submit bond or cashier's check in the amount of 10% of bid shall be cause for rejection of bid.. Successful bidder will be required to provide Payment and Performance bonds each equal to 100% of the Contract Sum. Surety on the bond shall be by a duly authorized U.S Treasury Backed company licensed to do business in the State of Illinois, acceptable to the Owner. Bids are subject to the Illinois Prevailing Wage Act. Refer to other bidding requirements described in Document 00 2113 - Instructions to Bidders 00 2113 and Document 00 3100 - Available Project Information. Submit your offer on the Bid Form provided. Bidders may supplement this form as appropriate. The Owner reserves the right to accept or reject any or all bids and reserves the right to waive any irregularity in bidding which, in their considered opinion is warranted.
