Each bid must conform and be responsive to all pertinent Bidding and Contract Documents. For information regarding this project, prospective bidders are requested to contact the District's Purchasing department by telephone at (909) 9898541 ext. 52009 or by email at CSDBids@csd.k12.ca.us Attention is directed to the requirements of the Information for Bidders regarding goals for Disabled Veteran Business Enterprise (DVBE) participation on this project. This project has a goal of 3% DVBE participation. Each bid must be on the District's bid form and shall be accompanied by the list of Designated Subcontractors, Non Collusion Declaration, Bid Bond Security, Acknowledgement of Bidding Practices Regarding Indemnity, DVBE Participation Statement, Site Visit Certification, Bidders Acknowledgment of Project Schedule, Bidder Questionnaire, DIR Registration Verification. Prime Contractors bidding this work shall require, pursuant to Public Contract Code article 4108, all subcontractors providing labor and materials in excess of $100,000.00 to supply an original signature and fully executed 100% Faithful Performance and 100% Payment Bond. All prime contractors bidding on this work must specify this requirement for subcontractor bonds in their written or published request for subcontractor bids. Failure to comply with this requirement shall not preclude the Contractor from complying with the subcontractor bonding requirements. Bonds are required for each specification section or combination of sections which exceed(s) the limit listed above. The practice of issuing separate purchase orders and / or subcontracts for the purpose of circumventing the subcontractor bonding requirement shall not serve to exempt the Contractor from these requirements. No payments, except for a reimbursement payment to the Prime Contractor for the cost of the Prime Contractor's own Faithful Performance and Payment Bonds, shall be made to the Prime Contractor until the Prime Contractor provides the aforementioned subcontractor bonds to the District. The District reserves the right to reject any and all bids and to waive irregularities in any bid The work under this contract consists of all labor, materials, equipment, and services necessary for the removal and replacement of flooring, wall base and accessories at Cucamonga Middle School as identified in the site map as specified in Exhibit A and reviewed at the mandatory Pre-Bid Conference/Job Walk. A. Moisture Testing Requirements 1. CONTRACTOR shall conduct moisture testing on concrete substrate surfaces prior to the installation of new Homogonous Vinyl Tile (HVT) and Luxury Vinyl Tile (LVT) flooring materials. 2. Testing shall be performed in accordance with the flooring manufacturer's requirements to determine if moisture levels are within acceptable limits for installation and warranty compliance as identified in EXHIBIT E (Product Installation Instructions). 3. The cost of moisture testing shall be included in the base bid amount and will not be considered an additional cost to the DISTRICT. B. Moisture Mitigation 1. If moisture test results indicate levels exceeding the manufacturer's warrantable moisture limits, CONTRACTOR shall be responsible for completing the required moisture mitigation before flooring installation. 2. The Bid Form includes a dedicated allowance for potential moisture mitigation efforts. CONTRACTOR shall not include cost of mitigation in the bidder's base bid amount. 3. If moisture mitigation is required, CONTRACTOR shall follow the same process as a Change Order for use of the allowance and be pre-approved by the DISTRICT in writing prior to any work proceeding. C. Moisture Testing Documentation & Compliance 1. CONTRACTOR must provide documentation of all moisture test results, including testing locations, dates, and findings. 2. Any mitigation measures implemented must comply with industry best practices and be approved by the flooring manufacturer to maintain warranty coverage. D. Demolition and Disposal 1. CONTRACTOR shall be responsible for the demolition and disposal of all existing flooring, wall base, and accessories as identified in the site map (Exhibit A) and reviewed at the mandatory prebid conference/job walk. 2. All removed materials shall be properly disposed of in accordance with local, state, and federal regulations.
