You are invited to review and respond to this Small Business Bid Solicitation (SB Bid) entitled 25-007 Loading Dock, Roll-Up and Fire Drop Preventative Maintenance and Repair at the DMV-Headquarters. Purpose and Description of Services: DMV Headquarters (HQ) Loading Dock, Roll-Up and Fire Drop Preventative Maintenance and Repair. Term: Thirty-six (36) months (see Schedule of Events for proposed start date of contract). See Exhibit A, Scope of Work, for a complete description of services. Estimated Total Contract Cost: $52,850.00 Bidder Qualifications A. Bidder must be certified by the State of California, Department of General Services (DGS), Office of Small Business and Disabled Veteran Enterprise Services (OSDS) as a Certified Small Business. B. On March 4, 2022, Governor Gavin Newsom issued Executive Order N-6-22 regarding Economic Sanctions against Russia and Russian entities and individuals. Economic Sanctions refers to sanctions imposed by the U.S. government in response to Russias actions in Ukraine, as well as any sanctions imposed under state law. By submitting a bid, the bidder represents that it is not a target of Economic Sanctions. Should the State determine, at any time prior to the execution of a contract, that the bidder is a target of Economic Sanctions or is conducting prohibited transactions with sanctioned individuals or entities, that shall be grounds for rejection of the bidders bid by the DMV. C. The bidder shall possess a valid C-28 or D-28 contractor license issued by the California Department of Consumer Affairs, Contractors State License Board (CSLB). All subcontractors that the bidder intends to utilize to perform work under the contract shall possess all applicable contractor licenses for the work they shall perform under the contract. The bidders contractor license number shall be indicated on the Bid/Bidder Certification Sheet (Attachment 1). The names of all subcontractors that the bidder intends to utilize and their contractor license number (if applicable) shall be indicated on the Bidder Declaration & Subcontractor/Supplier List (Attachment 4). All bidder and subcontractor contractor licenses shall be in good standing from the time of the bid opening through the term of the contract. Question Deadline 03/24/2025 at 10:00 AM PT Submit all questions via email to Allison Stanek Department of Motor Vehicles Phone: (916) 818-2848 Email: Allison.stanek@dmv.ca.gov Contractor's Full Business Name (to be completed by the DMV), hereinafter referred to as the Contractor, agrees to provide Department of Motor Vehicles (DMV) all labor, tools, materials, equipment, and travel necessary to perform preventative maintenance and unscheduled repair services on roll-up doors and sliding gates as listed in Exhibit H. In addition, the Contractor shall provide annual Drop Test Certifications for the State Fire Marshal inspections on the specified four (4) fire roll-up doors listed in Exhibit H, for the term of this Agreement. The Contractor is responsible for field verifying all dimensions and existing site conditions pertinent to this Agreement. The services shall be performed at: A. B. C. D. DMV Building East: 2415 First Avenue, Sacramento, CA DMV Building West: 2570 24th Street, Sacramento, CA DMV Building South: 2490 First Avenue, Sacramento, CA DMV Central Utility Plant (CUP): Sloat Avenue, Sacramento, CA Commencement of Work A. The Contractor shall commence work under this Agreement on the specified start date provided by the DMV Project Manager following the execution of this Agreement by the state. The DMV Project Manager shall provide the Contractor with written notification of the start date prior to the Contractor commencing work under this Agreement. The Contractor shall perform: A. B. C. D. Scheduled fire door Drop Test Certifications shall be performed on an annual basis (once every year) during the 1st, 3rd and 5th semi-annual preventative maintenance and shall occur between 5:00 A.M. and 7:00 A.M. All work dates and times must be approved by the DMV Project Manager before work is performed. Scheduled preventative maintenance services shall be performed during normal DMV business hours, Monday through Friday, 6:00 A.M. through 5:00 P.M, except state holidays, unless prior approval is obtained from the DMV Project Manager. The scheduled preventative maintenance services shall be performed on a semi-annual basis (once every 6 months), starting the first month following the execution of this Agreement by the State. All work dates and times must be approved by the DMV Project Manager before work is performed. The three (3) year schedule shall be provided to the DMV Project Manager, within thirty (30) days from execution of this Agreement by the state, for review and acceptance. Since the loading docks are in constant use, the Contractor shall use caution and allow for safe product performance during the work. Unscheduled repair services shall be performed during DMV business days and hours (Monday through Friday between 6:00 A.M. and 5:00 P.M.) and after DMV business hours. If the DMV Project Manager classifies the repair as an emergency, the service shall be performed on Saturdays, Sundays, or state holidays between 6:00 A.M. and 5:00 P.M. as determined by the DMV Project Manager. All work dates and times must be approved by the DMV Project Manager before work is performed.
