Receipt of Questions from Potential Bidders: March 21st, 2025 by 3:00PM Questions & Answers sent by OMH: March 27th, 2025 Select/Non-Select Letters sent to appropriate Bidders As soon as possible after the bid opening Bidder Protest Deadline: OMH must receive no later than 7 business days after date on nonselect letter Anticipated Contract Start/End Dates: 5/1/2025 - 3/31/2026 Scope of Work: 1. The scope of work below is for the replacement of some sections of concrete slab, at the school yard of 1500 Waters Place Bronx, NY 10461 for building 37. There is about 650 square feet of slab which will need to be replaced. These areas located on the School Yard of Building 37 have suffered settlement which has create a tripping hazard and they need to be corrected as follow: The defected areas which will be specified by the Plant Superintendent or his assignee, will need to be saw cut and jackhammered and demolished, o All cuts and jackhammering shall be done with caution creating minimal spreading of debris and overall dust controlled will be expected. o All debris shall be removed from the site as needed and immediately after being demolished. 2. These designated areas where this concrete will be removed or demo, will require preparation as typical, consisting of the following: o Proper tampering of terrain will be required; Terrain must be well compacted to provide uniform support to the slab. o 6" of 3/4 Gravel will be essentially required for the new 6" slab to be poured, o Proper tampering of the new set gravel will be expected. All base material must be well compacted to provide uniform support to the slab. o Rods of number 4 rebar will be lay out at 16" apart and in both directions, penetrating the existent and remaining areas, set with typical epoxy cement for proper grabbing. 3. A new concrete slab consisting of 6 inches thick and with a resistance of 3500 PSI of Concrete Mix will need to be poured into the prepared and cleaned areas and prepared as follow: o Proper pouring, with pocket elimination o Expansion joins will be use where necessary. o Control Joins will be use at a proper distance of no less than 5' apart for crack prevention, this CJs can be done by cutting no later than 5 days after pouring. o Avoid water splash on the slab, by having enough hands at the site to do the finishing on time and before concrete drying out. o Install Re-Entrant Corner where necessary to prevent corner cracking of the slab. o Finish all edges with proper edger tools as needed. o Sweep finish will be required to avoid slippery finish. 4. All forms if applicable, must be removed after complete drying of all surfaces and no later than 5 days after pouring. 5. The facility Plant Superintendent or his assignee, will inspect the job accurately before signing off on it.
