DESCRIPTION OF WORK Location 27th Street Project Length Proposed Improvement Remove existing pavement, cement stabilize existing base, construct new jointed PCC pavement, adjustment utilities, and install video detection system. IDOT Certificate of W. Wood Eligibility Required X Prequalification. If checked, apparent as read low bidders must file within 24 hours after the let ting an "Affidavit of Availability" (Form BC 57) in triplicate, showing all uncompleted contracts awarded to them and all low bids pending award for Federal, State, County, Municipal and private work. One original shall be filed with the Awarding Authority and two originals with the IDOT District Office. The Awarding Authority reserves the right to waive technicalities and to reject any or all proposals as provided in BLRS Special Provision for Bidding Requirements and Conditions for Contract Proposals. The following BLR Forms shall be returned by the bidder to the Awarding Authority: a. Local Public Agency Formal Contract Proposal (BLR 12200) b. Schedule of Prices (BLR 12201) c. Proposal Bid Bond (BLR 12230) (if applicable) Training Apprenticeship or training Certification (BLR 12325) (do not use for project with Federal funds.). C. Affidavit of Illinois Business Office (BLR 12326) (do not use for project with Federal funds) . The quantities appearing in the bid schedule approximate and are prepared for the comparison of bids. Payment to the Contractor will be made only for the actual quantities of work performed and accepted or materials furnished according to the con-tract. The scheduled quantities of work to be done and materials to be furnished may be increased, decreased or omit ted as hereinafter. Submission of a bid shall be conclusive assurance and warranty the bidder has examined the plans and understands all requirements for the performance of work. The bidder will be responsible for all errors in the proposal resulting from failure or neglect to conduct an in-depth examination. The Awarding Authority will, in no case, be responsible for any costs. expenses, losses or changes in anticipated profits resulting from such failure or neglect of the bidder. The bidder shall take no advantage of any error or omission in the proposal and advertised contract. If a special envelope is sup-plied by the Awarding Authority. each proposal should be submitted in that envelope furnished by the Awarding Agency and the blank spaces on the envelope shall be filled in correctly to clearly indicate its contents. When an envelope other than the special one furnished by the Awarding Authority is used, it shall be marked to clearly indicate its contents. When sent by mail, the sealed proposal shall be addressed to the Awarding Authority at the address and in care of the offi-cial in whose office the bids are to be received. All propos-als shall be filed prior to the time and at the place specified in the Notice to Bidders. Proposals received after the time specified will be returned to the bidder unopened. Permission will be given to a bidder to withdraw a proposal if the bidder takes the request in writing or in person before the time or opening proposals The proposed improvement, designated as Section 24-00411-00-RP, City Project 2024-11, 27 th Street Reconstruction consists of removing the existing pavement, cement stabilizing the existing base, construct new jointed PCC pavement, utility adjustments, and pavement markings. The work shall be performed in two separate stages with the Northernmost section being completed first. This work will consist of installing tie bars in accordance with Section 442 of the Standard Specifications. All longitudinal joints of the proposed pavement shall be tied to all existing and proposed curb and gutter and concrete pavements including center islands, medians, and pavement or as directed by the Engineer. This work will consist of installing welded wire reinforcement in accordance with Section 420 of the Standard Specifications. The welded wire reinforcement mat shall be positioned in the middle of the concrete pavement cross section and care should be taken to ensure the welded wire reinforcement remains in place during pour operations. This work will consist of saw cutting the existing concrete pavement full depth in accordance with Section 442 of the Standard Specifications. Full depth saw cuts along all existing curb and gutter, medians, and pavement that is to remain, will be measured for payment. All additional saw cuts, including relief cuts, will not be measured for payment. The soil cement mix design shall include the addition of 4 inches of addrock/millings to the existing soil base. Following the add-rock, 10 percent cement by dry weight, shall be mixed into the final stabilized base mixture. This will result in a mixture of soil and cement with a dry density of 119.7 pounds per cubic feet, requiring 9.0 pounds of cement per square yard, per inch of soil cement thickness. The base shall be compacted to a minimum of 95 percent of the maximum Standard Proctor, at the optimum moisture content of 12.9 percent, as determined by the ASTM D-698 laboratory compaction test. This work shall consist of placing cement mortar along precast, prestressed concrete bridge deck beams as required for fairing out any unevenness between adjacent deck beams prior to placing of waterproofing membrane and surfacing. This work shall consist of testing the ride quality of the finished surface of pavement sections with new concrete pavement, PCC overlays, full-depth HMA, and HMA overlays with at least 2.25 in. (57 mm) total thickness of new HMA combined with either HMA binder or HMA surface removal, according to Illinois Test Procedure 701 esting Using the International Roughness Index (IRI) . Work shall be according to Sections 406, 407, or 420 of the Standard Specifications, except as modified herein.
