The City of Mequon is seeking cost proposals for 3 storm sewer catch basin/curb inlet repairs at various locations in the City of Mequon. The repairs needed vary in scope and are detailed in the list of repairs for each street/area. Also included are storm sewer system maps showing the storm sewer catch basin/curb inlet repairs labelled with a letter. The storm sewer catch basin/ curb inlet repairs are also labelled with white paint in the field at each structure corresponding to the letters on the list and maps. Please see the attachments for the description and scope of the storm sewer catch basin /curb inlet repairs. The purpose of this Request for Proposal (RFP) is to solicit proposals from various contractors, conduct a fair and extensive evaluation based on criteria listed herein, and select the candidate who best fulfills the needs of the City of Mequon. If the organization submitting a proposal must outsource or contract any work to meet the requirements contained herein, this must be clearly stated in the proposal. Additionally, all costs included in proposals must be all-inclusive to include any outsourced or contracted work. Any proposals which call for outsourcing or contracting work must include a name and description of the organizations being contracted. Copy of contractor's liability insurance shall be included in bid proposal. No additional bonding required. The City of Mequon is seeking cost and qualifications proposals for storm sewer catch basin/curb inlet repairs at various locations in the City of Mequon. The storm sewer catch basin/curb inlet repairs needed are varying in scope and can be attributed to age and deterioration of the structures. All costs must be itemized to include an explanation of all fees and costs. Please supply a I ump sum cost for each of the numbered or lettered storm sewer catch basin/curb inlet repairs. Contract terms and conditions will be negotiated upon selection of the winning bidder for this RFP. All contractual terms and conditions will be subject to review by the City Attorney and will include scope, budget, schedule, and other necessary items pertaining to the project. All awarded storm sewer catch basin/curb inlet repair work can begin as soon as the contract is signed. It is anticipated that the contract can be signed by May 30. h The City will award as many of the catch basin/curb inlet structure repairs as the budget for this project allows. The priority of the structures awarded will be based on the condition of the structures. The completion date for this project is September 19 2025. Bid Description: Non-disclosure of the data contained within a proposal cannot be guaranteed because of Public Records Laws. All cost associated with preparation, submittal and presentation of proposals shall be borne by the proposer. The agreement resulting from this RFP shall be required to contain the following: "Provider agrees to indemnify, defend and hold harmless the City and its officers, officials, employees and agents from and against any and all liability, loss, damage expense, costs, (including attorney fees) arising out of this agreement, caused in whole or in part by the Provider or anyone for whose acts any of them may be liable, except where. caused by the sole negligence or will full misconduct of the City." The contract resulting from the award of this RFP shall commence on upon execution. The City of Mequon reserves the right to modify and waive any and all informalities or technicalities, or to reject any and all proposals and/or parts thereof, and to accept that proposal which it deems most favourable for the City. The City reserves the right to reject any or all proposals, to waive technicalities, or to accept the proposal deemed to serve the City's best interest.
