This contract is for the replacement of window panels at the former Cambria Rowe Business College building, located at 221 Central Avenue, Johnstown, PA 15902. The contractor shall provide all labor and materials for Window Panel Replacement in accordance with the Scope of Work. The replacement panels shall be an insulated panel with bronze painted heavy gauge break metal. The panels are of assorted sizes. The contractor is responsible for the correct measurement of the replacement panels. All firms or individuals responding to this Bid must submit complete responses to the information requested in this section and must note any exceptions to any information contained in the Bid. Bids will be evaluated based upon the requested criteria and be awarded to the lowest responsible bidder. Bidders should present information in a clear and concise manner, following the format indicated below. Bid forms are not provided. Responses to this section shall be typewritten and uploaded in a font not smaller than 11 point and may be double or single sided on standard 8 1/2 " x 11" paper. There is no limit to pages submitted for complete response to this section: 1. Statement of Qualifications: Contractor shall supply all company contact information, Current License(s), Staffing, Certifications, Copy of Current Liability Insurance, along with Contractor's qualifications and ability to successfully provide the services requested. 2. Name, telephone number, address, e-mail of the individual designated to receive all official correspondence relating to the project. 3. Describe the Contractor's qualifications and ability to successfully provide the services requested, including a description of prior experience 4. Provide a copy of PA Contractors license 5. Provide proof of insurance 6. Provide a statement on the when you may be able to begin the desired service. 7. Provide contact names and telephone numbers for at least three (3) individuals, municipalities or companies for which you have contracted similar work in the past year. 8. Provide a statement on the notification time necessary to begin execution of the desired services. 9. A certified check or bank draft, payable to the Johnstown Redevelopment Authority, or bid bond in the amount of 10% of the total bid amount shall be submitted with each bid. 10.
