All costs associated shall be included within the base bid for this trade contract. Scopes listed shall be priced in their entirety (inclusive of all labor, material, & equipment) for a complete & finished system per the contract documents. 1. All building structural concrete foundations, footings, and slab-on-grade as shown in drawings 2. Tilt-up concrete wall panels, including casting, erection, bracing, and patching 3. Building concrete including grade beams, curbs, equipment pads, and dock walls 4. Furnish and install all reinforcing steel, including rebar reinforcement, dowels, chairs, wire mesh, and related accessories 5. Installation of vapor barriers and curing compounds as specified 6. Sawcutting and installation of all control joints, expansion joints, and construction joints per drawings 7. All building-related concrete forming and removal after concrete curing 8. Site concrete work including sidewalks, curbs, curb and gutter, steps, patios, dumpster pads, transformer pads, light pole bases, bollard foundations, and concrete paving 9. Installation of light pole bases and bollards including setting, alignment, and grouting 10. Setting and grouting of all embedded items such as anchor bolts and sleeves provided by others 11. Provide layout for all work required under this contract 12. Installation of detectable warning surfaces at accessible curb ramps 13. All backfilling, compaction, and site grading necessary to complete concrete work 14. Repair and patch any concrete surfaces damaged during installation activities 15. All construction per City of Grove standards and specifications where applicable 16. Compliance with all applicable codes and regulations 17. Include all footing excavation. Stockpile footing spoils onsite as directed by CM. 18. Provide and install all rock base under interior and exterior concrete slab on grade. 19. Include all required traffic control. 20. Includes Concrete pumps if required 21. Includes all details as listed in the Request for Bid documents, including all applicable attachments. General Trade Inclusions: Line items listed below are typical in nature and shall be included (Furnish & Install) in the base bid IF supported and required by the contract documents, unless noted otherwise. Building Concrete 1. Provide layout, excavation (footing excavation), stockpile spoil removal onsite, concrete, reinforcing steel, expansion joint material, keyway, water stop, cure & seal, and miscellaneous as required to provide complete & finished foundations, footings, footing steps, grade beams, slabs, pads etc. 2. Include flowable fill under footings as required. 3. Furnish all Work required for complete and finished concrete foundations, slab-on-grade, stem walls, housekeeping pads within the building (for other trades), pour backs cut by other trades, and miscellaneous building concrete. 4. All concrete admixtures & fiber are included in this Trade Contract. Hot water, accelerators, and super plasticizers may be required (With prior approval from the structural engineer) to facilitate concrete pours. These additives are part of this trade contract. 5. Topping slab reinforcement to be supplied and installed by this trade contract. All topping slab concrete is supplied and installed by this trade contract (If applicable). 6. All drilling, epoxy, and dowels are supplied and install by this Trade Contract where required by construction documents. The mason will install all epoxied dowels for his trade contract 7. All concrete reinforcement is being supplied and installed by this trade contract. Rebar chairs, tie wire, and other misc. accessories are part of this Trade Contract. Trade Contractor shall stage, layout, place, tie (this Trade Contract furnishes tie wire), modify as required by field conditions, and install all concrete reinforcement, wire mesh, and chair. 8. Rebar tolerances are +/- _". 9. All masonry cast in place as well as precast rebar dowels shown (dowels, hooks, etc.) are to be provided and installed by this Trade Contractor. 10. Perimeter foundation insulation and freezer slab High Density Polystyrene insulation. Include insulation between interior and exterior foundation / concrete slab as required. 11. Subgrade for final grading and compaction purposes under all concrete areas will be within _". 12. Provide and install granular fill under all interior and exterior slab on grade concrete. 13. Review and acceptance of the all site conditions shall be the responsibility of this Trade Contract. Any discrepancies shall be reported to the CM before work of this Trade Contract proceeds. 14. Include all concrete joint work including, but not limited to, keyed construction joints, decorative tooled joints, expansion joints, expansion material, zip strips, saw-cutting, and isolation joints. 15. Provide all layout for this work including all batter boards & building corner layout. Shall have building corners established by a surveying company. CM to approve layout prior to placement. 16. Soil treatment for Termite Control to be included in this Trade Contract. 17. Provide and install all below slab vapor barriers per specs and manufacturers suggestions. 18. All recessed slabs (if any) and topping slabs shall be included in the Work. All grinding or patching of floors for preparation of finished flooring installation shall be the responsibility of this Trade Contract. 19. Any anchor bolts or embeds in concrete shall be installed (Supplied by others) in this Trade Contract. Tolerances for embedded items are +/- _". The CM will verify all bolt settings and any removal/re-install will be the responsibility of this Contract and approved by the Structural Engineer. Protect and clean threads/embeds during and after concrete pour. 20. Include furnish and installing non-shrink grout under all column, beams and embeds. 21. Slab flatness shall be per Contract Documents and meet quality restrictions by the flooring contractors. All work for grinding and patching prior to floor installation shall be included in this Trade Contract. 22. Receive, offload, store, stage, inventory, and protect all concrete embed items. 23. All temporary lighting needed to complete the concrete work is included in this Trade Contract. 24. All concrete pump trucks needed to complete the concrete work is included in this Trade Contract. 25. Trade Contractor work shall include all layout, excavation and grading work required for the thickened edges and thickened slabs. 26. Rub-finish all exposed concrete and concrete walls per contract documents and/or for a complete and finished assembly. 27. Provide chamfer edges for concrete that are exposed at 90-degree points. For example, concrete wall corners, door openings, windows, columns, benches, etc. 28. Cleanup and dispose of all excess concrete and waste of as directed by CM. Waste/excess concrete MUST be removed, not spread out and raked out into the dirt areas. The CM on site will provide a truck wash out location. Coordinate with CM. Trade contractor will provide wash out pit. 29. Includes lay out and saw cutting of control joints. 30. Provide any weather protection and dewatering required for the execution of the work provided under this Trade Contract. 31. Concrete pours and material deliveries shall be coordinated between Trade Contractor and CM (2) two days in advanced to coordinate with other onsite activities. 32. Remove all spoils to a designated onsite stockpile approved by CM. 33. Include all submittals, samples, mock ups, delegated design, certifications, or engineering as necessary to complete scope. 34. Include engineering of any forms, bracing, or supports as necessary. 35. Install all bollards supplied by others per detail at locations shown. Include footings, fasteners, and fill concrete if necessary. Dome top of bollards if required. 36. Include all vapor barriers or approved alternate. 37. Grouting of all column base plates, beam plates, etc. 38. Form and strip all concrete openings required by other trade contractors. This is to be coordinated by the CM. Protect all openings in floor per OSHA spray painted with "hole" and secured to slab. 39. Provide temporary fencing at all open excavations not to be immediately poured. 40. Clean and repair any damages, due to work provided in this Contract, to the streets or adjacent areas. Mud/debris on streets shall be removed and cleaned immediately by this Trade Contractor. 41. SWPPP. a. This Contract to comply with all directions and requirements for the Storm Water Prevention Plan. b. Any cost associated with rectifying damaged surroundings do to negligence by this Trade Contractor, will be the sole responsibility of this Trade Contractor. c. Any cost associated with rectifying damages to the wetlands areas do to negligence by this Trade Contractor will be the sole responsibility of this Trade Contractor. 42. Removal of all waste to an offsite location and dump/waste location permission is the sole responsibility of the trade contractors. 43. Include slab block outs for recess floor locations. Site Concrete 1. Sidewalk & concrete paving granular fill, steel reinforcing placement, wire mesh placement, thickened footings, keyed construction joints around columns, chairs placement, supports, slab dowels placement, expansion materials, keyway, saw-cut, anchor bolt placement, dowels, and vapor barriers if required 2. Ramps and stairs along with nosing's. 3. Install steel bollards, ADA steel post and ADA U-channel post for ADA signs. Bollards, steel post and U-channel post provided by Steel Trade Contractor. 4. Install dumpster posts set in concrete. Post provided by Steel Trade Contractor. 5. Pedestrian crosswalk striping. 6. Truncated dome ADA warning pads. 7. Include multiple mobilizations within this trade contract. 8. Sidewalk, concrete ramps, light pole bases, concrete stairs, loading dock, curb & gutter, transformer & generator pads, and miscellaneous site concrete. 9. Concrete admixtures. 10. Concrete reinforcement (rebar, WWF, etc.) is being supplied by this trade contract. Rebar chairs, tie wire, and other misc. accessories are part of this Trade Contract. 11. Expansion joints (Control joints and Construction Joints). 12. Base rock/sand for final grading and compaction purposes under all concrete areas. Any and all fine grading and final compaction of rock shall be completed by this trade contract. 13. Review and acceptance of the all site conditions shall be the responsibility of this Trade Contract. Any discrepancies shall be reported to the CM before work of this trade contract proceeds. 14. Cut/tool all control joints per plans and specs. Concrete joints requiring caulking will be by this trade contract as directed by the CM. 15. Layout, excavation, backfill, compaction, concrete, reinforcing placement, expansion joint material, keyway, water stop, cure & seal, and miscellaneous as required to provide complete & finished concrete areas, include epoxy & dowels. 16. Stage, layout, place, tie (this Trade Contract furnishes tie wire), modify as required by field conditions, and install all concrete reinforcement, wire mesh, and chairs. 17. Reinforcement tolerances are +/- _". 18. Coordinate with CM and owner provided third party testing agency for all testing. 19. Housekeeping pads for other trade contractors that may or may not be shown on the plans. 20. Backfill and compaction of work provided by this trade contract. 21. Temporary lighting needed to complete the concrete work. 22. Concrete pump trucks needed to complete the concrete work. 23. Include any and all costs & coordination associated with permits, fees, inspections or licenses (as required by your work), and as required to provide a complete & acceptable finished product. 24. Provide all curb cuts/asphalt/concrete paving cuts at street tie-ins, saw cutting, expansion/contraction joints. 25. Receive, stage, layout, protect, place, tie (this Trade Contract furnishes tie wire), modify as required by field conditions, and install all concrete reinforcement, wire mesh, chairs, and slab bolsters. 26. Include all layout, excavation and grading work required for the thickened edges and thickened slabs. 27. Rub-finish all exposed concrete and concrete walls per contract documents and/or for a complete and finished assembly. 28. Rock re-compaction (to 95%) density. 29. Batter boards and layout for this trade contract work. 30. Include all saw cutting and layout of saw joints. 31. The earthwork trade contractor has prepared subgrade base rock in exterior paving and sidewalk areas to within _". However, this Trade Contract shall be responsible for providing and installing up to 2" of additional base rock as an additional leveling layer. Notify CM of any discrepancies in subgrade prior to beginning operations. Once operations have commenced acceptance of subgrade is assumed. 32. Cleanup of all concrete "splatters" on masonry, structural steel, aluminum storefronts, curtain walls, hollow metal and other finished products as required. 33. Follow and include all cold/hot weather placement and protection procedures required by this scope of the work under this trade contract, the project schedule, or as directed by CM. 34. Provide any weather protection and dewatering required. 35. Temporary phones, faxes, and drinking water as required by trade contractor's employees and subcontractors. 36. All work included in this trade contract shall be according to the project schedule provided by the CM. 37. Set dumpster enclosure post supplied by Steel Trade Contractor. 38. Provide all overtime required to comply with/meet and maintain the project schedule. 39. Concrete trucks will only be allowed to clean out in areas designated by the CM. 40. Concrete pours and material deliveries shall be coordinated between trade contractor and CM (2) two days in advanced to coordinate with other onsite activities. 41. Remove all spoils to a designated stockpile approved by CM. 42. Responsible for any & all layout required.
