Site work and new construction of a medical facility in Kasabonika, Ontario. Completed plans call for the construction of a 3,444-square-foot, two-story above grade medical facility; and for site work for a medical facility.

The project is to design and construct a four unit (Fourplex) with each unit consisting of two bedrooms, 3-piece bath, laundry facilities, functioning kitchen, dining room and living room. Each unit will have its own outside exit with parking out front. Each unit will be approximately 80 m2 The Design component will ensure that the design meets all Sioux Lookout Building Codes as well as the Ontario Building Code. The Build component will be responsible for all required building permits including ESA, As well as the water, wastewater and electrical hookups. The following sections outline in general the scope of work and are not intended to provide every detail of the work to be performed by the Proponent to achieve the objectives in the contract. The scope of work items listed in this document shall not be considered absolute descriptions of the scope of work. The actual work will be developed further throughout the project with the concurrence of the Project Team. The Design Proponent's main roles and responsibilities are to: a. Provide the architectural design for review by the Owner. b. Complete the stamped architectural drawings by an Architect In good standing in Ontario, once approved by the Owner. c. Complete the stamped structural, electrical and mechanical design by a P.Eng. P.Eng. in good standing in Ontario The Construction Proponent's main Roles are to: a. Purchase and deliver the building materials. b. Apply and secure all building, electrical and ESA permits required for the construction of the building and connections to the water wastewater system. c. Complete the required earthwork d. Construct the turnkey building in accordance with the building permits and required OBOA building inspections. e. Purchase and install the following: i. Fridge ii. Stove iii. Washer iv. Dryer v. Dishwasher f. Connect the water and wastewater to the community system.
