The work shall be done in accordance with Section 408 of the State of Wisconsin Standard Specification for Highway and Structure Construction, 1996 edition. No bid will be received unless accompanied by a Certificate of Insurance indicating the Contractor has purchased and maintains insurances with the limits of liability and coverages not less than the following: Workers' Compensation $100,000 each accident $500,000 disease policy limit $100,000 disease each employee. Limits and coverages as required by State Statute. Please indicate on certificate if owner(s), sole proprietor, Member(s), and/or executive officer(s) are included under Workers' compensation coverage. Comprehensive Automobile Liability $1,000,000 Combined Single Limit Coverage to apply to all owned, hired and non-owned Vehicles Comprehensive General Liability $1,000,000 Per Occurrence Occurrence Form $2,000,000 Aggregate Including Contractual Liability Indicate on certificate if aggregate is a per job aggregate Excess/Umbrella Liability $1,000,000 Per occurrence $2,000,000 Aggregate. Excess/Umbrella coverage to be excess over employers liability, automobile liability and general liability.
