Any change in scope of work, services, products, and cost must be approved in writing by GBHC prior to the changes being made No work can begin without prior written approval from GBHC No work can begin without a preconstruction safety plan being completed and on file No work can begin until proof of required insurances have been verified by GBHC Business Office Contractor training may be required before work can begin Healthcare screenings and required PPE (Masks) will be determined by the rules established by GBHC, OMH central office and other agencies and departments Proof of specialized training, certificates, licenses may be required by all workers Prevailing wages apply to this job, please consult attached Wage Schedule Certified payroll will be required for invoicing Proof of cost of materials will be needed for invoicing. Percentage of markup will be capped at 10 percent GBHC will not be responsible for shipping, delivery, and handling fees for any items GBHC will not receive items purchased by the contractor Work hours are Monday through Friday 7:30 am to 4:00 pm. These hours may be adjusted to meet the facility operational needs and may vary depending on where work is taking place.
