Renovation of a mixed-use development in Clinton, Oklahoma. Completed plans call for the renovation of a medical facility.

This project is to have a Roofing company place a coating on the existing roof terraces to stop the infiltration of rain water into the building. Areas of the concrete Roof Decks (Terraces) will have to be roughed up for adhesion of the coating and during application sand will be applied for slip resistance. The Clinton Indian Health Clinic has reported leaks into the building getting carpet wet. Location- Clinton Indian Health Center 10321 N. 2274 Rd Clinton OK, 73601 This is a combined synopsis/solicitation for items prepared in accordance with FAR Part 13. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotes (RFQ) 246-25-Q-0067. This procurement is 100% Indian owned business set-aside under NAICS code 238160 with a size standard in millions of dollars 19.0. The Contractor shall place a coating on the existing roof terraces to stop the infiltration of rain water into the building in accordance with the statement of work and drawing at The Clinton Indian Health Center 10321 N. 2274 Rd Clinton, OK 73601. The period of performance for this requirement is 90 calendar from Notice to Proceed. The official visit will be June 6th at 10:00 AM at the Clinton Indian Health Center 10321 N. 2274 Rd Clinton, OK 73601. s are due no later than June 16; 2025 5:00 p.m. CST. The closing date for receipt of quotes is June 23, 2025 COB CST. The offer shall be received by email to jeffrey.earnestihs.gov or in a sealed envelope, addressed to this office, showing the solicitation number, your name and address, no later than 5:00 p.m. CST, on June 23, 2025. THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO PROVIDE A PROPOSAL CONTAINING: A) Firm, Fixed price quote on the SF18 form and a completed attached bid sheet. B) Technically specifications of the statement of work, acceptable materials list to be used. C) (2) Relevant past performance of this type of work, shall provide organizations name, point of contact, summary of project and award amount. D)Submit Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form (attached). Award amount is estimated to be less than 27,000.00. Offers shall be evaluated on an acceptable/non-acceptable basis. Unacceptability of "A" and "B" (stated above) shall render the entire quote unacceptable. Evaluation shall proceed as follows: 1. Quotes shall be ranked by price, from low to high 2. Quotes shall then be evaluated for technical acceptability beginning with the lowest priced quote 3. This process shall proceed until a technical acceptable quote is identified THE INDIAN HEALTH SERVICE SHALL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO MEET THE REQUIREMENTS IN THE STATEMENT OF WORK CONTRACT AWARD SHALL BE MADE TO THE RESPONSIBLE, RESPONSIVE OFFEROR OFFERING THE LOWEST REASONABLE PRICE AND MEETING ALL TECHNICAL SPECIFICATIONS. The technical evaluation shall be a determination based on information furnished by the vendor in the submitted proposals. The Government is not responsible for locating or securing any information which is not identified in the offer.
