Rehabilitate Runway 17-35 Lighting - Runway Closed Crosses, Low Profile Flag and Flasher Barricades,Remove Existing Electrical Features, Trench and Backfill - 24" Deep, 2" PVC Conduit, NO. 6 Bare Copper Counterpoise, NO. 8 Cable, Type C, 1/C, 5KV Power Cable, Junction/Pull Boxes, Bore 2" PVC Conduit, Furnish and Install 4KW CC Regulator (Runway Lights), Furnish and Install Pilot Controlled Lighting Module, Base Mounted LED MIRL (White/Yellow Lens) , Base Mounted LED MITHL (Red/Green Lens ), Base Mounted LED MITL (Blue Lens), Furnish and Install 12' Lighted Windcone, Furnish and Install LED Rotating Beacon Bidder must supply all information required by Bid/Proposal Form. This Project is subject to the following requirements as referenced in the Special Provisions: o Buy American Preference (Reference 49 USC 50101). o Foreign Trade Restrictions. o Davis Bacon (Reference: 2 CFR 200 Appendix 11(D)). o Affirmative Action (Reference: 41 CFR part 60-4. Executive Order 11246). o Govemmentwide Debarment and Suspension (Reference: 2 CFR part 180 (Subpart C), 2 CFR Part 1200, DOT Order 4200.5 DOT Suspension & Debarment Procedures & Ineligibility). o Government Requirements for Drug-free Workplace. o Nonsegregated Facilities Requirement (Reference: 41 CFR 60-1.8). Sponsor, in accordance with Provisions of Title VI of Civil Rights Act of 1964 (78 Stat. 252, 42 U.S.C. 2000d to 2000d-4) and Regulations, hereby notifies all bidders it will affirmatively ensure any Contract entered into pursuant to this Advertisement, Disadvantaged Business Enterprises (DBE) will be afforded full and fair opportunity to submit bids in response to this invitation and not be discriminated against on the grounds of race, color, or national origin in consideration for an award. o Title VI Clauses for Compliance with Nondiscrimination Requirements (Source: Appendix AofAppendix 4 ofFAAOrder 1400.11, Nondiscrimination in Federally Assisted Programs at the Federal Aviation Administration). o Title VI List of Pertinent Nondiscrimination Statues andAuthorities (Source: Appendix E ofAppendix 4 of FAAOrder 1400.11, Nondiscrimination in Federally Assisted Programs at the Federal Aviation Administration). It is the Department of Transportation (DOT) policy that DBEs, as defined in 49 CFR Part 26, shall have the maximum opportunity to participate in performance of contracts financed in whole or in part with Federal funds. All Bidders/Proposers shall make good faith efforts, as defined in Appendix A of49 CFR Part 26, Regulations of the Office ofthe Secretary of Transportation, to subcontract 1.63 percent of the prime contract dollar value to small business concerns owned and controlled by socially and economically disadvantaged individuals (DBE). IfBidder/Proposer for this solicitation qualifies as a DBE, contract goal shall be deemed met. Individuals rebuttably presumed to be socially and economically disadvantaged include women, Blacks, Hispanics, Native Americans, Asian-Pacific Americans, and Asian-Indian Americans. Apparent successful Bidder/Proposer must submit information concerning DBEs to participate in this contract. Information will include name and address for each DBE, description ofwork to be performed by each named firm, and dollar value of contract (subcontract). If Bidder/Proposer fails to achieve contract goal as stated herein, they must provide documentation demonstrating good faith efforts in attempting to do so. Bid/Proposal failing to meet these requirements will be considered non-responsive. For Contracts $50,000 or more, Contractor with 50 or more employees and subcontractors with 50 or more employees and who may be awarded a subcontract of $50,000 or more, will be required to maintain an affirmative action program within 120 days ofContract commencement. Bids/Proposals received more than 96 hours, (excluding Saturdays, Sundays, and holidays), before set time for opening bids as well as Bids/Proposals received after set time for opening ofbids, will not be considered and be returned unopened. Cashier's check, certified check, surety bound, or irrevocable letter of credit in 5 percent of Bid shall accompany sealed Bid/Proposal of each Bidder/Proposer. Deposits will be returned to unsuccessful Bidder(s)/Proposer(s). The City of Hooker reserves the right to reject any/all Bids and waive any/all informalities.
