The Cuyahoga Metropolitan Housing Authority (CMHA), on behalf of CMHA Police Headquarters is seeking quotes from qualified Parking Lot Repair. Refer to Exhibit 1 - Scope of Work for detailed project general notes, specifications, and drawings. QUOTE FORMAT All quotations should consist of, at a minimum: a. Attachment A - Bid Form b. Attachment B - ACE/MBE/FBE FORMS. The Authority will not execute a contract on the successful proposer's form of contract. Services shall not be performed for CMHA without an authorizing "Notice to Proceed" received by the Contractor by email. Each invoice or voucher shall, at a minimum, be supported by a summary of the cumulative costs, a description and date of the service provided, and the contract number.
