Esmeralda County (County) hereby invites qualified firms or individuals (Offerors) to submit written Statements of Qualifications (SOQs) to provide Engineering Services for the Esmeralda County Silver Peak Road Permanent Repair. The Engineering Services would include Design, Bidding, Construction Administration and Construction Management. This project is federally funded by the Nevada Department of Transportation. RSOQ documents obtained from any source other than the Esmeralda County Road Department may not be accurate or complete and each Offeror assumes all risks by its reliance on such documents. An Offeror who has not obtained RSOQ documents from the Esmeralda County Road Department will not be notified of Addenda issued by the County, which could contain material changes thereto (such as additions or changes to the specifications, extensions of time, etc.). The County is seeking a Consultant firm to provide to Engineering Services to include Design, Bidding, Construction Administration and Construction Management for the scope of work to repair the Silver Peak Road in Goldfield Nevada, damaged by hurricane Hillary for approximately 3 miles, more particularly described on page 35 - Exhibit A - Scope of Services. This Request for Statement of Qualifications (RSOQ) consists of the RSOQ cover page and Sections 1.0 through 2.0 of this document, including forms and attachments. The Instructions to Offerors are set forth in Section 2.0, and must be followed completely in order for the submitted SOQ to be given full consideration for award of a potential contract. The Statement of Work is set forth as attached in Exhibit A. The County's Sample Contract is attached to this RSOQ as Attachment B. Should an Offeror be selected for award, any resultant Contract is expected to contain substantially the same terms and conditions set forth in the Sample Contract. Any proposed modifications or exceptions taken to the terms and conditions of the Sample Contract are subject to review and approval by the County's contracting authorities. Offerors are advised to thoroughly read the Sample Contract to assure complete understanding of the terms and conditions. A successful Offeror will further need to provide appropriate insurance documentation in a form substantially similar to that provided in Attachment C at the time of award. Use of the word "contract" in this RSOQ does not imply any obligation on the part of Esmeralda County to enter into a Contract. The prime Consultant and its Project Manager shall comply with the registration requirements of Chapter 625 of the Nevada Revised Statutes (NRS), Professional Engineers, at the time of submission of the SOQ. Such individual(s) must actually be employed by the consulting firm that is submitting the SOQ for consideration by the County. The County reserves the right to declare the submitted SOQ as non-responsive if the Offeror fails to follow the above procedure.
