Work contemplated to be performed under this contract is as follows: o Remove and replace defective HMA surfaces and patches o Installation of emulsified asphalt sealant o Remove and replace Cement Concrete Curb Ramp o Remove and replace Cement Concrete Sidewalk o Remove and replace Cement Concrete Driveway Approach o Remove and replace Cement Concrete Extruded Curb o Remove and replace Cement Concrete Curb and Gutter o Property Restoration and Miscellaneous Work including Traffic Markings o Adjust/Replace Manhole Ring and Covers, Catch Basin Frame and Grate, Water Valve Boxes o Traffic Control and related services All work performed will be ordered, and as necessary further defined, through the issuance of written Task Orders. Each bid shall be accompanied by a bid bond in the form of cash, certified check, cashier's check or surety bond in an amount equal to at least 5% of the amount of such bid. Checks shall be made payable to the City Treasurer. Should the successful bidder fail to enter into a contract and furnish a satisfactory performance and payment bond within the time stated in the specifications, the bid bond shall be forfeited to the City of Bellevue. The City of Bellevue is also committed to complying with Title VI of the Civil Rights Act of 1964, that provides "No person in the United States shall, on the ground of race, color, or national origin, be excluded from participation in, be denied the benefits of, or be subjected to discrimination under any program or activity receiving Federal financial assistance." The City of Bellevue, in accordance with the provisions of Title VI of the Civil Rights Act of 1964 (78 Stat. 252, 42 US. C. 2000d to 2000d-4) and the Regulations, hereby notifies all bidders that it will affirmatively ensure that any contract entered into pursuant to this advertisement, disadvantaged business enterprises will be afforded full and fair opportunity to submit bids in response to this invitation and will not be discriminated against on the grounds of race, color, or national origin in consideration for an award.
