**As of August 15, 2025, the project was awarded to Petty Farms, with an awarded amount of $349,997.46.** furnishing of all necessary labor, materials and equipment to perform and complete the Lead Service Verification Project, Contract 2025-423, according to plans and specifications on file in the City Engineering Office, part of the office of the Director of Public Service, at City Hall, 120 Maple Street, Wadsworth, Ohio 44281, said plans and specifications being open for public inspection. The work covered by the plans and specifications includes furnishing all labor, equipment and the noted material to accomplish the hydro-excavation, verification, backfill, and surface restoration of water service lines. This improvement must attain Substantial Completion by December 31, 2025. All bidders must be qualified by experience and properly equipped to complete the work specified in the time allotted. The lowest and best bidder for this contract shall be the bidder whose bid is determined by the City to be the lowest and best bid. The low bidder will be selected based on the lowest unit price as described on the Bid Form. The City shall have the sole right to select the option and any alternative or combination of alternatives that best suits the needs of the City. Bid tabs will be posted to the City's website following the bid opening, no inquiries are to be directed to the City. The project is contingent upon the Wadsworth City Council's adoption of authorizing legislation. The City reserves the right to delay, modify, or cancel the project based on the Council's legislative decision. Each bidder shall be required to file with its bid, the following: A bid guarantee in the form of either: a. A bond in substantially the form set forth in section 153.571 of the ohio revised code issued by a surety company or corporation licensed in the state of ohio to provide said surety. Said bond shall be the full amount of the bid, or b. A certified check, cashier's check or letter of credit pursuant to chapter1305 of the ohio revised code. The amount of the certified check, cashier's check or letter of credit shall equal 10% of the full amount of the bid and the successful bidder will be required to submit a bond in the form provided in section 153.57 of the ohio revised code in conjunction with the execution of the contract.Said bond shall be issued by a surety company or corporation licensed in the state of ohio to provide said surety. 2. A non-collusion affidavit 3. A statement of bidder resources and experience bidders must comply with the prevailing wage rates on public improvements in medina county, ohio as determined by the ohio department of commerce, division of labor and worker safety, wage and hour bureau. No bidder may withdraw a bid within 60 days after the actual date of the opening thereof except as provided in section 9.31 of the ohio revised code. The city of wadsworth reserves the right to reject any and all bids, and to waive irregularities therein. Event Bid date Notes: Each bid shall be submitted in a sealed envelope clearly marked "Lead Service Verification Project, Contract 2025-423" and addressed and delivered to the Director of Public Service, City of Wadsworth, City Hall, 120 Maple Street, Wadsworth, Ohio 44281. Each bid must be submitted on the form provided by the City in the bid book and must be fully completed and submitted intact. Each bid shall contain the entire bid book with all addendums. Each bid must contain the full name of the party or parties submitting it. Additional requirements and conditions are described in the bid book. The work of this contract lies within the City of Wadsworth, Medina County, Ohio, and is located at various locations within the limits of the City's Lead Service Verification Project as illustrated on maps included in this bid book. Specific site addresses and additional details necessary to record the contractor's findings will be provided at the pre-construction meeting. The project consists of hydro-excavation, verification, backfill, and surface restoration of water service lines along with tabulation of the material type for future reporting to EPA. The water service lines being verified will be limited to those accessible within lawns or other landscape areas and will include service lines both on the community-owned side and on the privatelyowned side of the curb stop or other line of ownership demarcation. Water service lines within paved areas or otherwise requiring specialized surface restoration will not be performed under this contract. Question Deadline 08/04/2025 If a Bidder is in doubt as to the true meaning of any part of the plans, specifications, or other proposed contract documents, the Bidder may submit to the City Engineer a written request for an interpretation thereof. The person submitting the request will be responsible for its prompt delivery which must be at least 96 hours before the scheduled bid opening
