KUMC seeks to procure services for Kirmayer Parking Lot Improvements on the KUMC Campus in Kansas City, KS, which will fulfill the requirements as outlined in the attached Specifications and/or Contract Drawings. KUMC reserves the right to negotiate with selected finalists should it be deemed necessary. Scope of Work: This work requires a qualified asphalt contractor provide the following: The scope of the project includes a combination of mill and overlay (2") and full depth pavement replacement of the asphalt parking lots and drive around the Kirmayer Fitness Center Building at 2141 Olathe Boulevard and to the South of KUMC's P1 Parking Garage. There are two areas of mill & overlay which include the entry driveway into the area from Olathe Boulevard between the Kirmayer Building and the entry into P1 Garage AND the West portion of the parking lot facing Rainbow Blvd. The remainder of the parking lot will receive full depth pavement replacement as detailed on the plans with a Geogrd system. Selected portions of the existing curb and gutter within the project area are also to be replaced with new concrete curb and gutter. There is also a section of sidewalk and a light pole base that will require replacement of existing concrete with new. Bidders will be pricing this project as one project. Contractor to list what scopes of work that they do not selfperform on the included bid form. Failure to attend the pre-bid conference will result in rejection of your bid. Impromptu questions may be permitted, and spontaneous unofficial answers provided. However, bidders should understand that the only official answer or position of KUMC will be presented in writing. Supplier proposals: All suppliers must submit the following documents as part of their bid. 1. A completed and signed copy of page 3 of the KUMC RFQ guidelines document. 2. References, as noted below. 3. Completed and signed RQ-00106 bid form Rev0, which must include quotes for: a. The base bid and alt 1 for the Kirmayer Parking Lot improvements, to exclude bond costs b. Bond costs, to include both Performance and Public Works Bonds, for the base bid and alt 1 for the improvements to Kirmayer Parking Lot. Purchasing the above bonds in advance is not required; only providing the costs of such bonds is necessary to submit your proposal.
