Each bidder shall complete the nonocclusive affidavit, certification of bidder regarding equal employment opportunity, certification of bidder regarding nonsegregated Facilities, statement of bidders relationship, bidder's qualifications, representative/certificates/other statements of bidders and submit them along with contractors bid for performing the work and bid security in the bid package.
