The failure of a responding proposer (a "Proposer") to follow these instructions could result in Proposer disqualification from consideration for a contract to be awarded pursuant to this RFP. This document is issued by Polk City (the "City") which is the sole distributor of this RFP and all addenda and changes to the RFP documents. The City shall record its responses to inquiries and provide any supplemental instructions or additional documents pertaining to this RFP in the form of written addenda to the RFP. The City shall post all such addenda, together with any other information pertaining to this RFP, on the online page. It is the sole responsibility of each Proposer to review the website prior to submitting a responsive proposal (a "Proposal") to this RFP to ensure that that the Proposer has obtained all available instructions, addenda, changes, supporting documents, and any other information pertaining to this RFP. The City is not responsible for any solicitations issued through subscriber, publications, or other sources not connected with the City and the Proposer should not rely on such sources for information regarding the RFP solicitation. Proposers and any prospective Proposers shall not contact, communicate with or discuss any matter relating in any way to this RFP with any member of the Polk City Board of City Commissioners or any employee of Polk City other than the City Procurement Director or the individual designated above. This prohibition begins with the issuance of the Request for Proposal and ends upon execution of a contract. Any such communication initiated by a Proposer or prospective proposer shall be grounds for disqualifying the offender from consideration for a contract to be awarded pursuant to this RFP and for contracts to be awarded pursuant to RFPs or Requests for Bid that the City may issue in the future. City Manager and Staff Review: September 24 - September 30, 2025 City Commission Review: October 1, 2025 at 5:00 p.m. City Commission Selection: October 1, 2025 at 7:00 p.m. Questions regarding the RFP should be directed to Craig Fuller, City Enginer, in e-mail at cfuller@civilsurv.com; Modification to Proposals: A respondent may not modify or correct the Proposal any time after the Proposal Due Date, except in direct response to a request from the City for clarification. Revisions to this RFP: If it becomes necessary to revise or clarify any part of this RFP, the City will provide an addendum to each firm receiving this RFP. Expenses of Preparation of Proposals: The City is not responsible for any expense incurred in preparing and submitting a proposal. Rejection of Proposals: The City may reject a proposal if: A. The Proposer fails to acknowledge receipt of an addendum, or if B. The Proposer misstates or conceals any material fact in the bid, or if C. The proposal does not strictly conform to the law or requirements of rfps, or if D. The proposal is conditional, except that the proposer may qualify his proposal for acceptance by the City on an "all or none" basis, group basis, or a "low item" basis. An "all or none" basis bid must include all items upon which bids are invited. E. The City is under a pre- lawsuit claim or current litigation with the proposer. Event Bid Description: Submission of Proposal: Six (6) copies of the proposal must be submitted by U.S. mail or hand delivery to: Polk City City Manager's Office 123 Broadway Blvd., SE Polk City FL 33823 Attn: Patricia Jackson, City Manager Proposals must be submitted in written or printed form (five copies in bound form and one copy loose form) in a sealed envelope marked "Proposal - Engineering Services for Improvements to Cardinal Hill Wastewater Treatment Facility" and shall include all information required pursuant to this RFP. The envelope shall be marked with the name of the firm submitting the Proposal. The city requests that anyone submitting a proposal also submit a three-month proposal as well as a longer term proposal. (Neither faxed copies nor electronic submissions will be accepted.). The City may reject all bids whenever it is deemed in the best interest of the City to do so, and may reject any part of a bid unless the bid has been qualified as provided in paragraph D above. The City may, at its sole discretion, reserves the right to waive technicalities or irregularities, to reject any or all submittals, and or to accept that submittal which is in the best interest of the city, regardless of whether that submittal is the lowest cost proposal. The method of Contractor Selection has not been Determined at this time.
